Can you use this for different sizes? I'm looking for something that can cut a4 down to a6.

Most of these "semi automatic" cutters are not designed to be adusted HOWEVER if you can find a video showing its innards, a lof them DO have a fairly simple system of cutting wheels held in place with grub screws.
As such if this is one of those types, then you can manually adjust them, in your case you would still need three passes, once to cut down the center - and then twice more to cut the two halves into quarters.
A note of caution, if you are going to be processing a LOT of paper, this will quickly prove tiresome as you manually process each A4 sheet TWICE - including the "push and let go" second cut. No problem for a few sheets, not ideal if you have a ream to work through most weeks.
You might find it easier, faster and cheaper to buy a decent second hand guillotine - three cuts and you have just handled a whole stack of A4 - instead of 1 sheet.

They are fixed sizes to accommodate standard business card sizes. This video is the 85 x 55mm Galaxy Bussiness card cutter set up
