Derek was crying for Meredith because he told Mer shen she asked him if he was staying with Addison and derek said "she's my wife". After finishing the surgery he was in the elevator with Bailey and felt the magnitude of him making the wrong decison to end things with Meredith. He was in pain choosing the wife he's not in love wiith but decided to honor their vows when he and Addison got married.
