I've done something similar to this. Tie the folded "base" scarf over your head, but then tuck the tail end under at the nape. Then, tie the oblong. Then, take the side ends of the base scarf and tie them on top, either behind or on top of the oblong.

No. not all. Some (only a few) sects have the custom to do so. But the majority of women do not.
