Hello @MrBlueBullet, they can come loose without the proper tension from the spring. Every time we service a cylinder, we service the valve as well to help prevent this. Replacing the turn knob on a valve is an simple procedure that any diver can do in the field, and unfortunately, some times the wrong spare part will get used, and this is when we typically will see the turn knob start to become loose each time the valve is open and closed. A fun fact for you, just about all valves can accept a knob from other manufactures, however, the spring is specific.

I prefer to have a thread or two of the screw exposed, that way I can run my finger against it to make sure that it hasn't loosened.

I tried fixing my wobbly valve knob on the bus and was arrested for indecent exposure,😅
