I thought all vehicles in the Gerry Anderson universe were made out of pure Explodium, rather than being packed with 40,000 tons of TNT. It just saved time.
I think the accidental one was elevator car 1 personally. It looked way too erratic. And when 3 crashes you see the tires melting, and yes this could be added in later, but I’m pretty sure that in the wide shots it was *also* burnt, and it crashed during a wide shot if I remember correctly, so that to me makes it seem like an accident, whereas with 1 it swerved erratically and that seems like something that may happen if there was something really wrong with the guiding wire or if there was a bump in the road, and ESPECIALLY if it was both. The way that I personally think it went down is that Elevator Car 1 hit a bump caused by something or another which caused it to swerve which then made the guiding wire get messed up, and with it messed up it continued to swerve back and forth until the guiding wire snapped and Elevator Car 1 flew off the track as a result.
It is definitely Elevator Car 1 as at around 45:30 in the episode you can see that it's just below the nose of the fireflash, they cut before the accident happenes to show Virgil applying max brakes and the wheels exploding and then they cut back to the rest previous shot with Elevator car 1 going on it's own adventure. One interesting thing btw, at the beginning i didn't knew about the scene of Elevator car 3 crashing into the aircraft, because the entire scene is absent in the Italian version of the episode, in fact when the cars come out from the pod, you only see number 1 and 2. Why did they cut the scene? idk
Can we just take a moment to marvel at how Brains had devised, not only some of the most elaborate equipment ever to show up, purposefully suited to specific situations but that he also included redundant backups in case his original plan hit a snag? Genius!
I think the footage of the on set accident is the one where we see fireflash aborting the first landing over the 2 elevator cars. In the background, we can see the 3rd failed one unevenly bumping over the set, and the footage cuts to the shot of the plane blowing up just when it looks like the car is about to fall off the set. Doesn’t look like an intentional shot to me.
I think it was master control car because I seem to remember a scene after which virgil seems to steady the car implying that it went out of control for a second or two showing that it was incorporated into the episode because of its effectiveness

If car 3's crash was unintentional, that would change the whole chain of events, whereas car1's crash happened after the sequence up to that point had unfolded as planned. This was the most memorable scene for me as a kid. I think it's partly because the wheeled platforms seem to have little eyes, which gives them an anthropomorphic quality. (I was a sucker for anything cute)

From a storytelling point of view the crash of No.3 has more effect on the sequence because it establishes early on that that this is a difficult and dangerous operation and so significantly ramps up suspense throughout. For that reason alone my vote would go to that being the deliberate accident. The fact that building and exploding the other aircraft model would have been a significant extra cost, while simply rolling No.1 wouldn't have been, also suggests that the latter was the unplanned addition. (The large section of sacrificed aircraft probably wasn't out of stock as this was the first episode made and it was unlikely to have been built in that style or on that scale for previous Anderson series. It would have taken time to build and have been a real extra cost.)
I'm going for Car3 being the 'happy accident', only because its subsequent crash into the parked airliner has obviously been filmed on different film stock (the picture quality is measurably worse). So I'm guessing that was the scene filmed after the main shoot to seamlessly incorporate the 'happy accident' into the story - after all, Virgil could not have survived the Master Car colliding with a fully fuelled airliner (he says, trying to avoid the Explodium theory! 😁)
... incredibly, as I watched the scene again, and even knowing how it would end, I was a gripped by it all as much as when I saw it as a kid. I would still rate it as one of the all time best actions scenes in a film / TV show.

Remember reading it was car 3 that was accidental & the wire snapped causing it to run off set & everyone liked what happened so much they decided to include it crashing into the script & adding the 4th elevator car. The 3rd car crashing into the plane always looked out of place to me. I've always thought it was Virgil's car too due to the erratic movement of it. Maybe they were both accidental, but either way, made for great action & can't imagine the episode without those scenes.
It might interest you to know that when the Zero X crashed, it smashed into houses made out of LEGO! If you slow the footage down you will see bricks flying everywhere, it's quite hillarious.

Due to when the original series was made and released, it's a difficult question for me to answer. The chances of it being the Master Elevator Car or one of the radio controlled subsidaries is 50/50. Regardless, the sequence is epic and highlights the fact that even the most carefully planned rescue operations can go wrong, especially when deploying new equipment for the first time.

Fireflash had been in holding pattern for hours and the radiation safety factor expired. So it would be on low fuel.
