As i understand to get red skull "legally" you need to find green book on bookshelf and such it to reach secret room with thing that can be activated with railgun, then you need to suck all ghost into vacuum cleaner. After you done in that secret room behind a bookshelf there will be corridor leading outside, on the right behind the fence near the wall

To take the red skull, I just went up to the waterfall, started looking at the red skull and used the hook and that’s it. There was no mistake that you couldn’t take the skull.
