To me, this seems more like the main universe, so Bowser seems like the main culprit. But we can throw Bowser Jr. and the Koopalings in there as possible candidates.

The story behind the scars isn't even hard to guess. The only one capable of inflicting them on Mario is Bowser, and nobody else.

With the super Mario Bros movie, Power ups seem to put Mario in a state where the Injury Dormant but upon reverting back, they just kinda return.
