Demystifying PSA Velocity: Understanding, Monitoring, and Implications

    Estimated one in seven men will be diagnosed with prostate cancer in their lifetime. An estimated 220,000 men in the U.S. will be diagnosed with prostate cancer this year. Prostate cancer can be a complex issue for the patient and his family to understand. There are many treatment options for prostate cancer including surgical removal of the prostate, external beam radiation (IMRT / SBRT), proton beam therapy, cryotherapy, hormone treatment, chemotherapy, focal treatment, and even not immediately treating prostate cancer for low risk cases (Active surveillance). Early detection of prostate cancer is important. Most prostate cancers are detected when the cancer is still contained within the prostate gland, but some cancers are advanced and have moved outside of the prostate gland (metastatic). Each person with prostate cancer has their own unique conditions and situations that can affect treatment decisions going forward. Cancer staging radiographs (CT scan, bone scan, PET scan, etc) is often done by the urologist that performed the prostate biopsy to assess for spread of cancer outside of the prostate.

    Good information. I had a PSA test done back in 2017 with a result of 1.1 total. I’ll be 51 this month. Recently had another PSA test done a month ago and the results came back as 5.83 total. My brain is working on overload trying to understand how it increased so much in 5 years. Now have my first appointment with a Urologist tomorrow.

      I equate an elevated PSA to a check engine light coming on in the car. Only 15-20% of men with elevated PSA have cancer confirmed on prostate biopsy. Lots of options for elevated PSA including MRI of the prostate, checking advanced markers, recheck PSA, immediate random vs. MRI guided biopsy.

      @@theprostatecoach8058 Urologist requested another PSA. Had blood drawn yesterday and received results today. This time it was broken down differently. PSA total 2.3, PSA free .2 and PSA % free 9L highlight in red. Now I’m confused. Can you shed some light for me to best understand?

    I’m 60 y/o. My PSA last year was 3.27 ng/ml with a normal sized prostate at 23g and documented 0.5mm cyst. These findings got me worried. A year later, February 2024, my repeat PSA went up to 3.84 but my prostate increased to 33.9 g. The cyst decreased slightly in size. Needless to say it’s been a source of constant concern. My urologist didn’t seem to worry but told me to repeat the PSA in 3 months. May I have your thoughts please?

    So I’m aware of PSA density calculator. Is there a PSA velocity calculator?

      No PSA velocity calculator that I am aware of. Just look at your PSA from year to year. Most docs consider an elevation of PSA by 0.75 per year over a 2 year period to be concerning. Every case is different. Talk with your physician for guidance.
