    Let's face it ADT and Hormone therapy are CHEMICAL CASTRATION. Do a lot of research because the cure May be worse than the disease.

      @@theprostatecoach8058 The doctors were very pleased with the results. The catheter is removed 36 hours later, but her urine stream did not respond well at all, they think because the inflammation of the prostate due to the damage for the cryo- gave me Flomax temporarily and self catheterizing, which is not pleasant . My pee stream is week now, so I still have to use a catheter a little bit. I'm hoping that resolves itself. Any suggestions?

      @@theprostatecoach8058 everything went well a little bit of self-catheterization for a couple days to get everything going. They think they got full coverage and we're very pleased you know we're not using leeches anymore but it's a lot of guesswork still.

    Very simply you need to find a urologist that is willing to deal with the cancer in the least damaging way possible. Prostate removal should be last resort. They need to also see if the cancer is spreading or not. If it isn't, then it will mostly likely not need treatment. My great grandfather had prostate cancer and lived with it for 15 years and ended dieing from limes disease. Proton therapy is the new and best treatment for prostate cancer. Radiation has been proven to not just successfully kill cancer cells but also gives the patient a better quality of life, which Prostate removal could never offer. Many urologists forget there are worse things than death at times and become obsessed with saving the patients life without thinking of the consequences to his quality of life. Sure, removing the organ is the easiest way of removing all the cancer, but it will cause permanent erectile disfunction, and the patient will most likely need a catheter for the rest of his life. Yes, radiation also has potential consequences, but nowhere near as dramatic as the surgery. Most younger skilled urologists won't even give a man that option these days because they believe the patients quality of life would go down the drain, and the patient could end up finishing what the cancer would have done. So, quality of life is key for all cancer treatment, especially prostate cancer.

    Any chance for urosepsis when the stone(s) is lodged at the upper or lower pole of the kidney? that is, not in the ureter.

      Absolutely ! Stones in the kidney can block the UreteroPelvic Junction or a stone can block the calices if the stone is blocking the infundubulum from draining. Hope this helps !

      Gleason 6 by definition is not invasive. It can progress into higher gleason scores which is why you should be watched closely by your urologist.

    I’m 60 y/o. My PSA last year was 3.27 ng/ml with a normal sized prostate at 23g and documented 0.5mm cyst. These findings got me worried. A year later, February 2024, my repeat PSA went up to 3.84 but my prostate increased to 33.9 g. The cyst decreased slightly in size. Needless to say it’s been a source of constant concern. My urologist didn’t seem to worry but told me to repeat the PSA in 3 months. May I have your thoughts please?

    I had a euro lift like six years ago and about two years ago. I was diagnosed with interstitial cystitis and it’s definitely affected if I eat any citrus or spicy foods I know the whole list of things to stay away from, but I went months now without any irritation, I used to have to go to bed with an ice pack on my bladder at times when I would have a flareup but now I went months and months and months and I felt pretty much normal. I even had an alcoholic beverage about a week ago and it didn’t even affect me. I still stay to my diet and I am very active. I work out all the time, the only supplement I take now is just little aloe vera capsules because I heard it can help with the lining of the bladder. Any other information you have Dr. would be greatly appreciated is a lot of doctors in the medical industry right now is not really researching this like they could be there not, anybody trying to come up with a cure right now they feel like they can’t do it but I know you could rebuild the blinding of your bladder I mean I’m going months and months and months with no effects at all. I do know when I do eat something that irritates me I will pee more in the night from two times like five or six times and that’s just means it something it irritated me that I ate. I eat something spicy and I didn’t really mean to, but I did it, but most of the time I just pee a couple of times in the night and I don’t have any more pain in my bladder that keeps me awake any information you have. Please forward it to me.

    I had prostatitis for a year I tried all the medications they all did not work for almost a year I was having sleepless nights almost every day but I kept doing my research I never gave up tried western medicine they all did not work I do not what guided me to buy licorice root herb it only took a week that misery is gone I am now able to sleep

    I keep having e faecalis infection in the prostate. E faecalis lives in the intestinal tract. I’m always costipated so is making a lot of sense.

    ADT CASTRATION reduces sex drive... Yes..reduces libido to ZERO. Castrate your pet dog and see how much sex drive he has left. NONE Exactly the same effect on ADT once testosterone levels drop below castrate level. Don't forget to inform viewers THAT ADT causes significant brain fog and dementia, penile and testicular tissue atrophy, 4 percent bone loss every year, a twenty times increase in periodontal disease, increased diabetes, anemia, severe mood swings , serious increase of cardiovascular disease. It's a lot more than a few hot flashes and fatigue. And. ADT longervthan 18 months results in a two year reduction in life expectancy. ADT CASTRATION is cruel and barbaric treatment and the effects persist for years after treatment stops, and results in permanent CASTRATION in up to twenty percent of old men. Never mislead patients by telling them that side effects are generally well tolerated.. They are not. . Doctors administer these toxic drugs through coercion, deception, extortion, and intimation without obtaining FREE AND FULLY INFORMED CONSENT.

    My partner has a Gleason score of 3+4=7. He is going to have radiation. I think he should get a second opinion and look into the different test that could be done. I read about a test called PSMA which shows more than just a pet scan and also shows if the cancer has spread. I’m very upset. He is not well versed in medicine like I am and doesn’t want me to speak to the doctor. I don’t know why, but I feel as though I should get the whole picture.

    ADT doesn't reduce sex drive...it destroys it completely.. ADT makes erections and orgasms almost impossible ADT isn't temporary..After 18 months on ADT CASTRATION, 75% of men will NOT RECOVER their testosterone even after a median time of FIVE YEARS and up to twenty percent of older men will NEVER RECOVER ABOVE CASTRATE LEVEL. You neglected to inform your viewers that ADT reduces life expectancy and that 18 months or more in ADT CASTRATION reduces overall survival by two year's. These are facts the doctors and big pharma do not want disclosed because it would harm the sales of these outrageously expensive toxic drugs. FULL DISCLOSURE is essential before men can give FREE AND FULLY INFORMED CONSENT.

    I had focal brachytherapy in 2020 for low grade cancer. 3 years later I have a large high grade recurrence and the only viable option for a possible cure is salvage prostatectomy combined with ADT and more radiation. If I had to do it all over again I would have had a prostatectomy instead of high risk surgery with high risk or morbidly.

      sounds like you were one of the unlucky ones , as the cure rate percentage is highest with this type of treatment with low grade cancer , was it because you had focal treatment and not had the whole prostate treated ? there is about a 2% chance of infection after the biopsy and i had 2 weeks of various antibiotics because of it unfortunately i was the 2% , thankfully there are many treatments available for recurrence , good luck in your journey .

    Great video very thorough I think you explained everything very well. I am post up for my robotic process. To take to me at this point and still have the foley catheter in. I am a physician myself and I would recommend this procedure to anyone that is in good health that is serious about getting rid of the cancer

    I think one advantage is you get it all over and go onto the mend. If it goes well then your worst day should be right after surgery. Other forms of treatment may involve side effects getting worse over time rather than better.

    No thanks!, I have low grade prostate cancer and I have opted for active surveillance rather than radical invasion treatment. Rather have 10 years of quality of life than 20 years of misery! But that's my choice. Have altered my diet to many fruits and vegetables, fish. Inclued tomato sauce every day ( for lypocene benefits which have been shown to kill prostate cancer cells). Drink green tea every day and increased my vitamin d supplement from 1000 IU's to 2000 IU's. Continue to exercise daily with weights and hill sprints twice weekly. So far, feel fit and healthy with no symptons. I'm hoping by cancer at least remains low grade Gleason 6 by adopting above measures.

    Thanks Doctor Stiles for this presentation. I was recently diagnosed with stage 4 prostate cancer with a Gleason score of 8 after my biopsy. I have a PET scan scheduled for next week and a Radiation Oncology initial consult the end of this month. Trying to determine my best course of action for a 67 year old. You are definitely helping me with this.

    Biopsy may actually spread cancer cells before therapy even begins. Why else do most patients have recurring tumors years after radiation or prostatectomy.
