Worked with an ex-marine meth head for a tent rental place once
We were taking apart GIANT home Depot style shelving units for our shop.
He unbolted the shelving unit from the ground and it came crashing down. I had to tuck and roll, most literally. Of course it was a big joke for everyone except for me.
But now that same guy is facing federal charges for doing treason on Jan 6.

Maybe the forklift operator was not wearing a seatbelt? otherwise no other OSHA violations were observed from the video. Was it over capacity? Was the operator trained and certified under 1910.178(l)(4)(iii)? Don’t throw the OSHA act (5)(a)(1) because that’s lame. It was stable so it when the guys helped they weren’t in violation of the 1910.176(b) standard.

I work in a rigging barn and do this shit all the time. No violations there. Anybody that says different, has no clue about this type of work. Tag line needed. Not when the load is on forks. Hardhat for operator? Not when he's under that cage.

Improper safety equipment.
The trailer does not have blockers or whatever those are called.
With the quality of the video, the wheels on the forklift look like they need to be changed.
Forklift driver didn't use the horn when moving forward.
Endangering co-workers by helping them with something that will have a good drop.
Having someone that close to the forks while they are lowering down.
More along the lines of this video being osha. It should be (X) forklift violations.
