Non regulation trailer. Using a forklift. Flat tire. Humans being too close to a moving forklift. No PPE. The list goes on!

Improper safety equipment. The trailer does not have blockers or whatever those are called. With the quality of the video, the wheels on the forklift look like they need to be changed. Forklift driver didn't use the horn when moving forward. Endangering co-workers by helping them with something that will have a good drop. Having someone that close to the forks while they are lowering down. More along the lines of this video being osha. It should be (X) forklift violations.

That 2 guys not wear helmets and when there is loading or unloading must be trained rigger is there, so it should be violation.
