I want to thank you so much for pointing out the importance of urinary sphincter length to probable incontinence result. It will help in making my treatment decision.

    Dear Doctor:
How soon after a Robotic Assisted Radical Prostatectomy can I resume my maintenance BCG treatment for my NMIBC safely? What is your best suggestion on this?
    Thanks.

Hi Dr. Like what you said. I am an advanced prostate cancer patient who just went through robotic RP 2 months ago. The only adverse thing is that 5/14 LNs were invaded (0.1mm to 2mm). PSA is 0.5 ng/ml. The oncologist recommends wait and see before giving me any adjuvant radiation and/or hormone. I think he is right that all I need for now is just rest and then deal with those LNs later. If PSA continues to rise, then a good PET scan can be ordered for IMRT. What do you think? Thank you.

@DR Dan Thank you very much for sharing your thoughts and info. Much appreciate it. This dog dewormer drug is quite interesting and some papers stated that it works well with Vitamin E. Three month of medication doesn’t impose toxicity risk. May worth a try. The other quite interesting topic is the Bipolar Androgen Therapy (BAT) and I may want to try one by one. Then Theranostics with 177Lu or 225Ac is my last resource down the road of PCa.

@DR Dan Thank you for writing to me. I am at the beginning of castration resistant so may try BAT and intermittent ADT now. My urologist may start the 2nd line of hormone therapy with Rubeqa soon. I studied the design-wormer and it appears to work on some PC. Question is that in Canada we only have Vermox 500 mg and Mebendazole Polymorph 100 mg. Both require prescriptions. I will try both during the intermittent ADT. Hope to see some controls. Good luck on your journey of recovery and never give up hope. Cheer!

cFlaxseed, white button mushrooms, and a vegan diet have been
      shown to slow prostate cancer.
The difference between no treatment and the use of radiation
and androgen depravation appears to be about an 8% increase in survival time
      for those who are treated
Is it worth it for an additional 8% considering the side
      effects of treatment?
Would it be a better plan to use the dietary measures
mentioned above to treat recurrent prostate cancer?

@@tnvol5331 how is that working for you? How is your b12 level on vegan diet? We tried a less invasive focal treatment called IRE and are focusing on whole food plant based diet. Worried we are not doing enough.

@@sandraredmond4812 I did have 35 radiation treatment but with a vegan diet, white button mushroom powder, flaxseed, and apigenin all of which have been shown to slow down cancer. I take a 500 unit B-12 each week and my B-12 level when tested is at the lower end of the normal range.

@@tnvol5331 how did you feel after the radiation? Any long lasting or late surfacing side effects ? What is apeginin? My husband is following the care oncology protocol which includes daily baby aspirin and metformin as well as Claritin. How do you take the b-12. My husbands red blood cell level is low and it concerns me. He is taking b-12 supplements but maybe he should be taking a higher dose ?? He did an IRE ablation of the index tumor. May need to also do radiation. He is concerned with side effects. He is only 60. Too young for this ! As anyone is! It’s dreadful.

@@tnvol5331 we have also switched to a vegan diet. Cut out dairy at least mostly. I make my own breakfast cereal with hemp seeds , flax seeds and basil seeds and throw in some nutritional yeast ( for the b12) and cinnamon as well as coconut and nuts and cocoa nibs. Mix with water or nut milk. Top with sugar free coconut yogurt for probiotics and some blueberries. Delicious and nutritious.
