    Doctor, from an anesthesiologist, retired after 43 years of giving anesthesia as both a CRNA and an anesthesiologist, I’ll say that you do an awesome job with your videos. Nice job explaining life in the OR. Nothing like a granola bar and a Mountain Dew at 0230, on call.

      @@michellelyman7092 haha always gets me that some find Mountain Due and such to be too much... That's coming from someone who can smash a 4 Pack (2 Litres) of Energy Drinks in a day, don't do that anywhere near as often as when I was younger but sometimes 🤣

      @@santinojozefmiller7721 To be honest, I don't care if the anesthetic doctor is Jewish, German or whatever, the main thing is that he's not aggressive and lets me sleep in peace and wake up again peacefully. My last general anesthetic went so wrong that I postpone it before the next one.

    I will never forget my first pedi-Code, 40+ years later, he was a boy, 15 months old, went with the doctor to tell the parents that after 90 minutes their child had died; back when I did volunteer Fire & EMS. RN x 30 years, last 21 in the OR. I loved nights and emergencies as you find out so much about people by the way they handle them & treat others. I would have loved having Anesthesia residents but we weren’t big enough in my last hospital. Your attitude and your knowledge will take you far. 👍🏼👍🏼🙏🏽

    As both medic and patient that has been intubated 7 times, I love Anesthesiologists. Always made me feel more comfortable know that some with airway training was handling my airway; since I'm a grade IIA airway. Thank you for the videos and showing the Anesthesiology side of medicine. You rock and will excel!

      @@endlessrandomness1057 In fairness, surgeons could hardly be relieved in this way. Unless it was predetermined that certain surgeons would handle certain aspects, even then it would be quite unsafe to change the primary surgeon ad hoc.

    I’m a Maternal-Infant nurse of 30 years. I especially appreciate nurse anesthetist & Anesthesiologist because when emergencies arise, they are really the key doctors that coordinate ( anesthesia, blood etc)…in the OR. Thank you !

    I've had 11 back surgeries, and have met some really special people. You have to be very strong in this field. To see such an excited , warm, caring doc this , is really nice. I've seen these docs n all kinds of situations and they are very special people to me. Thank you for all you do.

      @@Tappedbravo549 RNs are essential people on the healthcare team. I absolutely couldn't do my job without help from RNs in the preop area, OR, and post-op care. Being a nurse is incredibly hard work that often goes underappreciated, and comments like this don't help.

    Thanks for the interesting videos!! I recently had my first (and hopefully last 😂) surgery and was so very grateful for the anesthesiologist, surgeon, and nurses that helped me. I was very nervous and the anesthesiologist took the time to help me feel much better before the surgery. I love your positive attitude. Your work and knowledge is changing people's lives for the better every day!

    I just had sinus surgery..... my 12th surgery overall..... anesthesia is my fave part of surgery. I always get great anesthesiologists. Thank you for always keeping us safe and making sure we wake up!!

    I love how this profession is constantly monitoring screens and numbers and making slight changes, following procedures, etc. But are still a doctor but not actually performing surgery. Very interesting

      not all specialties are as "flashy" (for lack of a better term), but equally important! Radiologists and pathologists are very very "underrated", too!

      @@merdzky098 it's cool because they are all baseline doctors at the end of the day. Anesthesiologists are flashy to me because they are like masters of drugs, chemistry, medicine, keeping people alive, and using the body as a human biology experiment and chemistry set.

    Thankk you to all the nurses, doctors, maintenance staff, food service, and all the other people that were on the Covid front lines. Yes, I did say maintenance and food service because they too were on the front lines keeping the hospitals running while the doctors and nurses worked at saving lives. You were, and are, amazing people.

    Im a vet tech and it's amazing how the two worlds parallel each other. Watched your prepping for pediactric sx video, just about every single thing and equipment is exactly .what we use. Thanks for sharing this side of medicine with the general public.

    Makes me so sad. My one day old son was transferred after birth to Children's Hospital in Denver. He received cpr and was revived during open heart surgery but died the next day. Was a complete surprise so very overwhelming. Had the same anesthesiologist who did my epidural for my c section 14 months later. He asked how the last year went and I said my son died. He said, " I remember that baby." He totally changed his demeanor with me.

    Several years ago I was blessed with a wonderful, caring anesthesiologist for my eight hour cyto-reductive surgery for metastatic appendix cancer. When the local apostolic pentecostal minister came in to have prayer with me and my family before I went to the OR he asked if he could stay with us while we had prayer. Judging by the fact that I was up in northern Indiana in the middle of Amish and Mennonite country and his last name I think he has had a Christian upbringing, could very well be Mennonite. He even went the extra mile and turned the radio on to the local contemporary Christian station while he and his assistants were getting me all ready and starting my anesthesia. The last thing I remember is hearing the song "I can only imagine" playing, next thing I remember is waking up hours later. He even reassured me about an epidural since I had a previous bad experience with the procedure.

    Hey Dr. thank you very much for your videos and for what you and your team do. I have learned so much, and have been more comfortable knowing what training and what you go through. I have suffered two STEMI 100% occlusive heart attacks in eight months recently. I don’t remember a thing. You helping me understand what you you guys go through has made me appreciate you even more than I do. I mean I have appreciated you guys so much already and even more if that’s possible. Thank The hospital again for letting you do this.

    Another insightful video Dr. Thank you for sharing. After the Corps, one thing I learned doing 24 hour shifts as a LA City paramedic is never say the word quiet out loud . LOL..You lucked out lol...My Grandfather was a surgeon and was a field surgeon in WW2. Spent a lot of time watching him work growing up. I went from being a medic to becoming a forensic PA..But keep the video's rolling. Thank you for your service as an MD..

    Hi Max,you are awesome! I have a lot of Doctor friends and I'm a Professor of romance languages so I teach them Spanish .I developed a course in Spanish for the medical profession. I love your videos because I have had 4 back surgeries and I know all the medical vocabulary because of my latin language studies. You really are an awesome person and a great doctor. I could see you as an instructor because your speech is perfect. All my best to you.

    This video made me reminisce about my residency. You may have a "toxic" duty wherein you get absolutely no sleep/rest OR a "benign" duty where everything goes smoothly throughout the day and you end up having CBR (complete bed rest) 😉

    Hi Max,
    I've really been enjoying your videos since I discovered them yesterday. My interest in medicine is that of a lay person--I wish I could have become a doctor (I'm a musician) but my talents are elsewhere. I've had many surgeries for various things. I had surgery for a broken wrist not long ago and since I was being given general anesthesia I asked the anesthesiologist about what it involves, what the risks are, etc. I have great admiration for the amount of extra training you go through with the patient's safety foremost. I have a request for a future video: can you explain why sometimes patients get nauseated and vomit nonstop after they come out of anesthesia? This only happened to me once, but it was miserable! Once again, I love your channel!

    I'm glad you mentioned at the NG tube because when I was in Germany studying there, I had an emergency appendectomy and they put one in and it was awful and I don't know why but now I know why

    Hi Max, a couple things I wanted to share with you. 1. I do not think that your profession is a “lifestyle specialty.” I want and need someone who is well trained, knowledgeable in any anesthesia emergency and well rested to care for me as I “sleep” during my surgery. After all, every breath I take, every move I make, you’ll be watching me. (Little humor but very true.) My life is literally in your hands. Thank you for all you do keeping us safe during surgery. 2. You mentioned in your most recent video, 24 On-call that you had to lay off the Haribo gummy’s. Nah, you look very similarly today then you did when you made this video. Your face is filling in as a mature man. 😊 Spoken like a mama. 😉

    Anesthesiology residency in my country is quite tough, seems u had a chill 24 hour call. We usually have around 34 hours call in nepal and handling around 10/12 cases in that periods along with icu rounds and vent as well as cvc insertion all day long . Nice to see u get a good 4 hours sleep

      Hi there, for your M2 year the most important focus is doing well on Step 1 (obviously you know this). Once you hit 3rd year, focus on doing well throughout all your clinical rotations. I know that seems obvious, but the importance in anesthesia is that it shows you get along well with providers from all services. As an anesthesia resident/attending, you'll work with providers from all services so it's good to indicate you can already do this well as a med student. Your surgery and internal medicine rotations are particularly important because in a sense, the anesthesia knowledge base lies at the intersection of those two specialties.
      For the end of 3rd year / beginning of 4th year, definitely try to do at least two anesthesia rotations so you can really be sure that it's your career of choice! And not just for you-- this is important to demonstrate on the interview trail that you've spent enough time to prove to your interviewer that it's an informed decision.
      While I did a fair amount of research (because I enjoy it), I don't think research is necessary for anesthesia.

      Things only get better as you move along in your training. Obviously pre-clinical years are just a grind. MS3 is tough but at least you're with patients. MS4 is usually pretty great. Then PGY1 you're finally a doctor, and PGY2 you get to actually do anesthesia! That's as far as I am, and it's the best haha.

    In South africa. Its illegal to work a 24 hour shift as an anesthetist due to the critical work they do and decisions that are made. I'm surprised you guys do this in the states

    Awesome video! Your dog is so cute! Looks like ours. Sounds like your in a awesome safety hospital! Its so huge! I've had 4 sinus surgerys both sides. Always wondered how the estimation works with a surgeon up in your field. Thanks . Plus are u who gives the sinus blocks when done so theirs no pain or does ENT do it? Its great to have the stimulation twice a week. Awesome. I can imagine your 1st ped stimulation would be terrifying. Loved the back pack full of so much! When I worked as a nurse we never had anesthesiologists respond to codes. I'm very impressed to know they do now. Very informative. Thank u so much

      Glad you found this video interesting Sherry! Anesthesiologists can learn various types of nerve blocks for structures around the face, but from what I've seen, it's typically the ENT who will do those. At least at my hospital, the level of collegiality and collaboration between ENTs and anesthesiologists is very high (our department has co-authored several textbooks with ENT attendings) so there's a lot of teamwork. It's pretty cool and would be a fun video to make of an ENT surgery some day.

    Awesome video as always, I want to know how anesthesia is sometimes referred as one of the lifestyle specialties when you work the same hours of our colleagues in surgical specialties 🤔
    My main concern is about the working hours and the on call shifts after finishing residency..

      Hey Fouad, thanks for watching and commenting on my videos so consistently!
      I think that on average, anesthesia residents work fewer hours compared to surgery residents. However, we still work 60-70 (or more) hours per week, plus call once per week or more, which is not consistent with a lifestyle specialty IMO.
      As far as number of hours worked and number of call shifts after residency, that really depends on where you decide to work and how you negotiate your contract. I don't know a ton about that process, but it sounds like you can pretty easily negotiate to take less call/hours but that comes with a pay decrease. Really depends on the specifics of the job- it's hard to generalize across the entire field.

      Hi George, great question. I'd say there are some characteristics that are uniquely important in anesthesiology as compared to other specialties. Those that come to mind are:`
      - Good communicator, specifically with surgeons and other members of the team in/around the OR
      - Calm disposition, particularly in the face of acute and severe problems
      - Sociable (at least for short periods of time lol), especially when it comes to quickly establishing rapport with a scared patient 10 minutes before they undergo a huge surgery

    As someone interested in pursuing anesthesia or IM, I find your vlogs very informative. Thank you for taking us with you. My aunt who's a psychiatrist in the US is convincing me to take IM b/c she says anesthesiologists are always on-call. Plus there's the concern about CRNAs? What do you think about these issues? I personally love anesthesia even tho it can be nerve-wracking, the fact that you're in charge of a patient's physiology in the OR, and that there can be a learning curve with the hand skills.       I was actually very interested in pursuing IM, but anesthesia won out because I enjoy the OR so much, love the opportunity to help nervous patients feel a little less worried before surgery, and I like the pace of anesthesia ("95% boredom, 5% sheer terror"). The concern your aunt has about call really depends on what kind of practice you'd like to do. You can find anesthesia jobs that require no night or weekend call, but of course you take a relative pay cut for that. Really depends what you're comfortable with. As far as midlevel practitioners, you should ask your aunt what she thinks about the growing number of NPs who are practicing hospital medicine. Midlevel practitioners are present in most fields, so anesthesia isn't unique in that sense.
      The bottom line is that if you love anesthesia, you should do it. Just my 2 cents!

      Hi Jordan, yes I was actually able to get a decent amount of rest during the shift when I recorded this video. I usually have a pretty constant energy level during the day when I'm at work, but it's common for me to come home feeling quite tired.
      As far as caffeine, I always have a cup of coffee in the morning, and occasionally will have a 2nd cup in the afternoon if I'm on a 24-hour call.

    I had an emergency appendectomy however mine had also burst so I had peritonitis and I’d like to say that is effing painful even after everything is done but it’s scary as hell and I had not eaten anything all day because it took that long for them to figure out it was horrible. And I was in my early 40s.

      Hi Bonnie, thanks for watching! We have "call rooms" that have a bed (clean sheets put on daily) and a computer where we can easily access electronic medical records. I will say that sleeping in the hospital is never nearly as peaceful as sleeping at home :D

      @@Hi-jx5gu Hello! Residency works differently in my country, so I finish medical school august 2022 but you don't start residency until 2 years post med school graduation. You have to complete internship in OBGYN, PAEDS, Medicine and Surgery then do a Senior House Officer year before residency. I'm currently doing my final year elective in Anaesthesia tho!

      @@lisamorrison4636 that's cool! I have worked with doctors that graduated from the UK residency system, and they praise it for getting you well exposed before deciding a specialty. My country adopted the American system of 4 or more years of residency which I personally prefer tbh. It must be torture to rotate in different specialties when you already know what you want lol.       Hi Eloim, glad to know you're excited about applying to anesthesia! I appreciate the nice feedback.
      I think the majority of anesthesia programs take a wholistic look at your application, so Step 1 score isn't the only deciding factors, but all the other components of your application. I'd advise focusing your efforts on things you enjoy, whether or not they're directly related to anesthesiology. The program directors who will be interviewing you know that you're not just a future anesthesiologist, you're a person with interests outside of that. You'd be surprised how much non-anesthesia stuff came up during my interviews. Anyways, point is that you should do what you enjoy as far as research / volunteering / coursework etc.
      The specific things you should make sure to do are at least a couple anesthesia rotations to demonstrate that you've clearly gotten exposure to the field and have made a decision based on that exposure.
      Keep in mind that most people choose anesthesia late (3rd or even 4th year) so few people have CVs that are filled with anesthesia-specific stuff, unlike derm / ENT / ortho etc.

    I wish I could have had you as my anesthesiologist! I am crazy anxious before surgery, so much so, that my mouth gets too dry to swallow - which in turn causes me to become even more anxious. In my personal experience, nurses would not ask for anything to help with the anxiety. I almost walked out (gown intact) because I was so uncomfortable.

    Awesome video! I’m an aspiring IMG from the Philippines hoping to get matched in Anesthesia next year. Thanks for sharing raw experiences, it’s been really helpful for me since I have yet to get my Anesthesia US clinical experience next year. Would really appreciate also if you could share insights on how a non-US IMG could get into a program. Is Mount Sinai one of those hospitals they consider “IMG-friendly” hospital? Keep it up, Doc!

      Hi Michael, glad you found the videos helpful! In addition to having good USMLE scores, I think a great way of maximizing chances of matching as an IMG is to do away rotations at hospitals where you hope to match. I know that's hard with COVID right now, but any way that a program can get to know you (and vice versa) is helpful. Mt. Sinai anesthesia has had IMGs but I'm not sure proportionally how many. I believe there's a spreadsheet out there called Match a Resident that identifies programs that tend to take more IMGs. Hope that helps!
