Normally there is vasoconstriction of efferent arteriole but Ace inhibitors cause vasodilation of efferent arteriole that's why ace inhibitors increase gfr in unilateral ras...

@@n.s.studytime i agree vasodilation of efferent arteriole cause decrease in gfr but in ras there is too much decrease in gfr beacuse blood is not going towards parenchyma of kidney...so there is only one method to increase in gfr of ras PATIENT to vasodilation of efferent arteriole... Bro do comparison .. between the ras patient and ace inhibitor use in ras patient..

U/L ras..main problem is hyper-reninemic hypertension.. hypertension is due to increased renin only..no other way to treat this htn other than ACE OR ARB
If you compare it with b/l ras..there is no pressure natiuresis.. leading to increased salt and water retention ultimately blocking renin release. ace contraindicated there..to prevent renal shutdown.
