I am requesting to make this a habit and accept the fault when it happens during the year and during the appraisal that mistake would count lesser since you admitted it yourself rather than someone else coming and blaming you. Apart from this I also recommend that one should display their value with their work such that the organisation fears them leaving if they don’t get a good appraisal.
I’m sure that if the fault discussion comes up during the appraisal there would also be a justification about it, as to why it happened and what corrective measures were taken. The idea is that in mind of the management the employee should not be seen as someone who hides bad news or escapes fault. This would make them more positive or less negative about you in general.
