    Believe it or not, I'm a Pharm.D. and I've studied a lot and prepared notes of Heart sounds, but all of it went over my head just after few hrs.     Sir can you kindly describe the physiology of splitting of S1 in asd... Because as I know that in asd there is loud S1 and splitting of S2 through out the respiration... So if you kindly describe it then the concept will be more clear to me... Thank you

    Why in VSD is wide & variable splitting of S2 & wide & fixed splitting of S2 in ASD? In both condition volumes are same. Can you please elaborate more?

    One doubt, you said that S4 represents the contraction of left artia and falls before S1, my question is, the sequence i understand is first there will contraction of the atria following it the blood will rush from the atria to the ventricle ( but the rushing of blood from atria to ventricle is S3) so shouldn't S4 be heard before S3 and not before S1? As the mitral valve will close only after the blood rushes, so S1 will be heard after S3.

      1st ,I'm sorry to reply to your comment which is not directed to me.
      Now ,let's reason it out.
      We understand that both s3 and s4 only occur in either pathology or in given physiological conditions like pregnancy(s3)
      S3 occurs rapid feeling of ventricle, meaning the ventricles take lesser time to fill .
      Now ,S4 is described in relation to s1 not s3 because all cases with s4 will never have s3,
      Since s4 only occurs in non compliant ventricle whereas s3 occurs in compliant ventricle.
      So it will be wrong to say s4 comes after s3 yet in conditions with s4 you'll not get s3 ,
      The most constant variable is s1 and s2 that's why the description of both s3 and s4 are done in relation to s2 and s1 .
      That's my understanding ✌️🙏.
