The cart, balloons, him wearing black but others are wearing white all the way to someone offering him white clothes at the end. He is on his own path, his own journey, searching for sanity, relief and peace.
When he says “put my make up on”, he is talking about his “fake face”.
The people in the background wearing white are people who aren’t showing them true selves. They are still carrying their burdens (black balloons), but they are presenting their “fakes faces.”
NF is wearing his normal clothes to symbolize that he is showing his true self.
Carrying his burdens and dragging a cart through the grass and mud.
His life is hard, and he doesn’t want to put a “fake face”on, but he has to at his shows.
That’s why at the end he is sitting in the cart overwhelmed and someone comes and brings him white clothing because he still has to do his tour, and still has to deal with all his other burdens at the same time.😮😮

The amount of heat and passion he puts in his music. How much of nf is laced throughout his whole discography. He can't be beat. I've said before that nf has work to do before he can be called goat, but that was before I really listened. They're isn't anybody like nf in any genre. This is greatness.

Yo, I just wanted to say that NF has a LOOOT of symbolism in his videos. Almost everyone of his vids on this album is connected. The people in the white clothes, the balloons (obviously) they all kinds the together. Just watch them in order and you will see what I mean. Don't concentrate on ND but the backgrounds. Trust me you will start to see the connections in his vids. He uses symbolism in a different way and it's crazy.
The symbolism with his balloons is his depression. Or his burdens (he refers to them in like his second verse in the beginning to middle I believe) I havint figured out what the white dressed people are I have a few ideas but nothing concrete.

Therapy Session was 2016 and was referring to how his music is a therapy for him as he cites at the end of the title track. He actually had to see a therapist last year in 2018 which is why he did not release an album then.
In psychiatry and psychology they often ask you to imagine your problems are balloons and try teach you to let them go. Some even do this in practice taking you to an open space to let each balloon go and fly away as a technique to help healing by physically letting the balloons go and hence, metaphorically, letting go of one’s problems/burdens/demons.
