I think the whole Mark of Cain/Demon Dean thing was so brutal because Dean was so strong when Sam did his whole demon blood/Lucifer/Soulless Sam thing and then he was strong for Castiel when Castiel did the whole I'm-Your-New-God/Purgatory thing. Watching Dean fall like that after being so strong for everybody is probably what made it all so brutal to watch.
