What the secret ending actually meant is that LN2 is a prequel to LN1. In the secret cutscene, Six loses her soul, which looks like a glitch. In order to stay alive, she has to kill people and take their souls. That's why she ate the rat and the gnome in LN1. Also the glitch-soul-six looks at a picture on the floor of the boat you are on in the first game, as if to tell Six to go there.

To explain the secret ending, 6 drops mono because she sees his face and is reminded of the thin man or she just pissed he broke her music box, when she leaves the tv her shadow forms into glitch 6 that looks on at the mall and makes her Hungary setting up the first game, the idea of this is she is stuck in a time loop because she let mono fall he will always become the thin man just like she the lady, everything in the game has already happened, the teacher getting her neck stuck in the vent which is why it's long, the doctor having ash falling off him cause he's already built and the hunter already being shot in the face because you shot him.
as for the thin man, he never hurts you he's chasing you away from the tv, from 6 when your close you hear static similar to when you put a mic next to an amp because there the same person he's trying to stop mono from ending up the same way he did and when he's older it all begins again
as for 6 she travels the first game trying to fill the void and hunger her soul left behind and every time she eats you see it in the background watching until finally she eats her future self and restarting the loop she broke all the mirrors becouse she new if little her found them then it will never end

I think the secret ending isn't just that i think that its telling us its a prequel
Think that was six’s soul I believe. She can absorb people’s power and use it but at a cost. She couldn’t get through the portal without the powers of mono. She let him go because he is always getting her into trouble which is bad because she is worried for her own survival. And 6 is the lady and as you know mono is the stick boy (forgot his name lol). This is only a theory (not mine) but still it explains a lot
