Well, its about circuses that twisted people would create really back in the day (around the time of the world wars). They would steal children from their families and beat them or maybe worse, making them into attractions for people to laugh at (e.x. Len and Rin were literally stitched together, and Kaito was forced to eat human flesh until he developed a taste for it). When Mieko looks into the hole, its referencing how in real life, the owners had people do that, since what they were doing was illegal and they didn't want people to take pictures. Also, if they rebelled, they would get burning acid thrown into their face.
After my friend Larryn watched this, she had a dream where her and our friends ( including me ) switched the places of the characters. Me and larryn was in the place as rin and len. D:
