Yes possible. The specification of the Fan which I am using is below
Voltage: 12 V
Current: 0.15 amp
Power: 3/2.5 W
The relay showed in the connection schema is different from the actual physical one. Are you able to show the actual physical connections with the physical components?
Actual relay only has 3 pins, IN, GND, VCC, and 3 insertion points, NO, COM, NC.
I am not getting the similar relay component to draw the circuit

HI. MY RELAY IS NOT GETTING A SIGNAL TO TURN ON AT MY SET TEMPERATURE, IT JUST KEEPS RUNNING. THE REST OF THE CODE WORKS EVEN THOUGHT ITS DIFFICULT FOR A BEGGINER LIKE ME.
AM I SUPPOSED TO REPLACE THE "FAN_PIN" ON THE SETUP SEGMENT OF THE CODE?
