The thing about joker still retaining speech and consciousness in his titan form, I think boils down to this: other victims of the titan formula lost their minds during the transformation process. I think because Joker already lost his mind long ago, it doesn't affect him. He's already crazy. The titan formula only made him stronger.

Joker is the only one to retain his mind while on Titan, Batman is able to resist the transformation for atleast 10 minutes before curing himself while everyone else changes right away. In city Batman is poisoned by Joker's blood which is bad due to Titan but Batman is still infected despite having been injected with the cure. It is also implied Ivy was hit with Titan (this may or may not be true) and she had no negative side effects. Then in the Arkham Knight for some reason everyone gets weird effects from the Joker's Titan blood causing mind and body warping. All this from what was just a monster steroid. Plus Batman was the first one to be injected and yet he was also the first to be cured but is still sick? The games are fun but they often do not make much sense due to poor writing.
Since no one seems to understand, I'll explain: Joker kept his mind while on Titan because he has a stronger mind than his henchmen. Batman was resisting the transformation through willpower but still needed the antidote. Joker was dying in Arkham City because the Titan overdose poisoned his blood, increasing its toxin levels. Batman was poisoned because his antidote only reverses the Titan transformation, where he and Joker's henchmen only got a regular dose. A different cure was needed to treat the side effects of the overdose. They had different writers for Knight instead of the ones for Asylum and City so I won't bother with Knight.

What gets me is how come Batman's Hand didn't come off after using the explosive glue on it, i think it was because of the gauntlet Batman was wearing.
Given the hundreds of skulls he probably drove those knuckles into in that one night, Batman’s gauntlets have to be stronger than just about any metal.
I love how Joker's monologue "...giggling in a corner and bleeding..." is a direct callback to Scarecrow's third interaction, where we see the different versions of Batman giggling and crying. It really highlights Asylum's theme of madness, and how close to insanity Batman really is.
Seriously. I'm replaying the game and got to this point but I just can't beat it because of the shitty close range combat mechanics. It's especially hard in this one when they have knives, tasers, AND guns. And I'm only doing this replay on normal smh

I'm going to be very honest. Yes Arkham Asylum is a magnificent game! But it was kinda disappointing in my humble opinion to watch the Joker inject himself with the titan formula. The Joker is a fantastic villain who doesn’t need that stuff, it’s better to watch him go against Batman with his wit, his insanity and his evil plans. Just look at Heath Ledger’s Joker in The Dark Knight! He completely broke Batman mentally in the end. So in my humble opinion i don’t like it when he does this kind of stuff.

Batman's borderline supersoldier in the Arkham series. He even resisted a lot of fear toxin dosage enough to drive 10 men insane. I think this is the why Joker didn't even bother trying to shoot Batman another dose of Titan.

How did joker survive? Like in general how did he survive right after this transformation long enough until the venom killed him slowly from sickness. When he transformed he clearly has exposed ribs, torn up skin with exposed muscle and veins, the spikes out of his back and arms. When he went back to normal size he should be practically a skinned piece of flesh with a really big stretched out blanket of loose skin on him covered in holes.
