My learning experience:-
WRITING STEP BY STEP
1. Identify the key issues.
Highlight the Keywords/synonyms/referents in the case note.
2. Identify the relevant,semi-relevant & irrelevant case notes.
Whatever we have highlighted is the relevant information .(i.e,with Keywords/synonyms/referents such as all the concerned medications).
3. Transform each case note into a simple sentence.
4. Check with Grammarly
5. Transform simple sentences into compound sentences.
The connected simple sentences can be transformed into compound sentences.
6. Check with Grammarly
7. Transform simple sentences & compound sentences into complex sentences.
2 or 3 complex sentences are enough.
8. Check with Grammarly
9. Write a story (speak out) of the patient.
write as we are giving the handover. (i.e, explain the details like who is the patient, what happened,what is happening, what medications have been given, what is to be continued )
10. Check with Grammarly
11. Segregate the case notes/sentences based on the key issues.
If there are multiple issues, segregate the sentences based on the key issues.
12. Arrange the sentences/points according to sequence (cohesion).
Arrange the segregated sentences in an order.
13. Write the different paragraphs.
Sentences should be logically connected. Connect them with the introductory phrases, Connective words and Sequential linkers.
There are 2 approaches in writing the paragraphs.
a) Chronological:-
*here we will be writing day by date,major dates will be considered.
*one paragraph will include only one time frame work.
b) Thematic / issue-based:-
If we have 2or 3 issues, we will be ordering the paragraphs as per the issues .main issue will come first, then 2nd, then 3rd.
*each paragraph will have one theme.
14. Start writing the letter in the writing format.
A. Address
B. Date, Salutation & RE line
C. Introductory / Purpose paragraph
D. Body Paragraphs
E. Request/ Recommendation Paragraph
F. Sign off
