Never expose the entire head, neck and upper chest for a TMJ projection. Collimate to a square 8x8cm maximum.

Exposing the whole head and neck to radiation is not only bad for the patient, it is bad for the quality of the image. Collimate the projection to about 8x8cm. I used to put 6 of those exposures onto a 24x30 cassette.

I agree. Though a straight lateral is possible, it is never done routinely unless the the mandible is required for assessment. Using 20 times more radiation than required is an enormous but regular crime.
