I mean, this is the only part of the film that is a bit of a stretch. That doctor and detective didn't notice the vial of blood in his hand? At the same time, given the situation Vincent is in what else was he supposed to do I guess.
The character of the doctor (Xander B) actually knew that "Jerome" was a bit suspicious for years. But that's the point; he LIKES him. And for whatever reason or reasons, he actually helps him when it matters the most. Tons of foreshadowing set this up, too. "Jerome the metronome" is one of many. 😉 Xander's character knows.
The doctor noticed or knew what was going on and went along, he tells Vincent at the end that "his son was a big fan of his" meaning of what he's doing and fighting for his dream. So he's telling his son that Vincent is beating the odds. Its a tender moment at the end, and even tells him to stop shaking his lizard with the wrong hand.

When he got up he kept the sample stuck in his arm and then changed It for thé other Blood sample
