So to be a medical dermatologist, does one need to attend a medical-dermatology residency, or can one became a medical dermatologist with a dermatology residency alone?
You can be a medical dermatologist by attending a derm residency alone. Most med derms out there did that. I just love internal medicine, so ended up training in that specialty too, and still practice medicine to this day. For people who are interested in practice med derm, you just have to make sure you go to a derm residency that sees a lot of med derm. And if not, you may want to consider a med/derm fellowship after residency.
Med derm residencies are probably a little bit more competitive for 2 reasons: There are only 5 programs in the country, and each program only has 1-2 spots. Also, you have to impress two different admissions committees at the same place to get into a med/derm program. the IM program is just as important as the derm program, so you really need to be impressive on both sides! However, the people interested in med/derm programs is smaller, so that helps decrease the competitive-ness.

Good grades, research experiences, leadership, and volunteering. Most programs are looking for well rounded applicants!
