I think it is profound that Fat’s conclusion upon the Doctor treating him like a human being, is that the Doctor is also insane. As if empathy is conditional upon a shared view of reality in a way. And that since the Doctor is similar in his view and willing to hear Fat out, that allows him empathy and thus makes him insane. Or it could be the opposite, where one leads to the other. It’s thematically significant either way, in my view.
