I was diagnosed with diminished ovarian reserve (DOR). How many IVF cycles do I need for 2 children?

    Today, I’m answering a question from Diana who wrote in to Ask The Egg Whisperer. She has diminished ovarian reserve and wanted to know what she should do to have 2 children. Diminished ovarian reserve is a diagnosis often related to age, where the ovaries have a diminished supply of eggs.
    Here’s what she had to say,” Just got diagnosed with DOR (amh 0.37 and 8 follicles) and a large fibroid (9cm, asymptomatic until recently which is why I booked my appt) and trying to figure out next steps. How would you advise to proceed, as I was not able to find that much info about this (dor + fibroid). How many cycles do you think I would need for 2 live births?”
    I’m going to assume that Diana is about 37 years old (the average age of patients I see), and here’s what I have to say:
    Knowing that you have eight follicles with a nine centimeter fibroid, I would say this let's take things in stages.
    Stage one is the diagnosis phase. iIt’s important to take quality supplements. We know another part of the diagnosis is the fibroid.
    Stage two: Create an IVF plan that is based on your diagnosis. I’d focus on creating embryos and freezing them. And if you're a patient of mine, we would talk about something called PGT a, I know this sounds like an alphabet soup of fertility, but it stands for pre-implantation genetic testing for aneuploidy.
    Stage three: We would then plan your embryo transfer. That would include a myomectomy to review and remove your fibroid. We don't want that fibroid to compete with the growing embryo or pregnancy, and that might put you at higher risk of having preterm labor and preterm delivery.
    Talk to your doctor about the IVF protocol they're going to use for you. Talk to them about how many cycles they think that they're going to need to do for you to reach your family size goals, and then go for it.
    And before you do surgery, make sure you're seeing a surgeon for that fibroid that is super excited to restore your uterus.

    I have been diagnosed with hormonal imbalance and as such, I have irregular menstral cycle. Also my egg reserve is decreased to just 3 left. 2 on the right and 1 on the left. I am totally heart broken, I will as ready to undergo any process to have my own kids.

    I so appreciate you and everything you do. Sometimes when I hear you deliver the hard news to someone about them not being able to have their own biological children you use alternative wording suggesting that they can "build a family in another way." I just want a flag that for some people genetics/ancestry and two people creating someone biologically together is only what they consider to be family. Adoption/egg/sperm adoption would not feel satisfying or like family. It would feel more like giving up and perhaps more devastating than accepting that one would never have children. I just wanted to put that out there in case it's helpful to realize that for some people that alternative may not mean what it means to you. 🙏🏻💙

    Hi Dr.Aimee, my husband (42m) and I (36f) will start our 6 cycle of ivf+icsi. The last 5 cycles we did we the same doctor and clinic, and only one of the cycles resulted in an embryo transfer but with a negative pregnancy result. After the last cycle, the doctor started to mention the option of egg donor. My hubby has low sperm count, and other hand me having endometriosis and apparently diminished ovarian reserve. In your opinion, when would a doctor start suggesting the option of egg donor?

      Thank you for this question. I mention egg donor when: I can't get a mature egg, or a mature egg to fertilize, or an embryo to grow. I will also mention that I will keep trying different ways to improve egg quality+sperm quality to see if I can overcome the challenges and if after 1-3 cycles, we're still in the same place, I like to talk about other options.

    Hi dr, I have a question? I had surgery in my uterus with polyps! And after a month a tried IUI but failed I did 4 times and tried IVF first was failed 2nd all my eggs was abnormals! Was canceled! My doctor told me I have DOR and better to buy donor eggs. I am only 34

    Can ivf stim reduced AMH level. Mine was 20 pm/l british Unit to only 3.4 in less than 1year. Tested in 2 different lab last year was 20 pm/l. So frustrated and my ivf cycle cancelled due poor response 💔? Please advise me what I should do next

    Dr. Aimee, can I take DHEA supplements with a DHEA(s ) level of 621? I’m 41 with E2 of 13 and FSH 28 .

    Any possibility with amh level 0.06 was originally 0.01 so it went up which is awesome but still very low

      I got pregnant with amh of 0.2. Don’t even worry about 1.1!

    Im 33 and my fsh level is 24. My Dr. didnt even bothered to suggest me any supplements, she directly said that my changes of getting pregnant with my own eggs are minimal and that i shouldn't waste money and time on ivf...

      You can’t know how many eggs you have left. Even a woman after menopause still has 100’s or maybe thousands of eggs. There are just increased chromosomal abnormalities & less eggs released to fertilize each month. Maybe check out Dr. Mark Sklar too. These doctors cannot give you medical advice here. Read “It Starts with the Egg”. 🩵
