Addressee could be written as (The scholarship committee, Stefan Banach Scholarship Programme) while some recommendation letters can also be addressed to,”To whom it may concern”. Most importantly, it should be on the letter head on the organisation or institution and be properly signed; also, the content should reflect your research, professional or academic abilities. Congrats! You need to write to the polish embassy for visa appointment date, that is, if you also have your admission letter from the uni you want to study in. Meanwhile, you should also proceed to federal ministry of education and foreign affairs for the authentication of your academic documents; don’t forget to legalise the authenticated certificate at the Polish embassy too. I think instead of uploading your CV; you can write your experiences with relevance to your course in the space provided in the form or have them written in your recommendation letter.

No worries! You can always change your university of choice. It doesn't affect your application.
