The only thing in the outdoors that rivals the danger of lacking competence is having too much confidence. I haven't watched this in a while but I believe in the story he states that the couple was very experienced. They showed up for a day on the river and saw that it was raging but thought, "well, we're really experienced, we'll be fine" and that's all it takes out there.
