A baby is breathing in a very asynchronnous manner while on ventilator, we try to sedate with high doses of midazolam, and then fentanyl, but asynchony persists, her RR is 106 as shown on ventilator monitor. We give atracurium and now the baby develops a significant drop in saturation. What should we do in such circumstances, give atra or avoid, or try something else.
@@childhealth5049 great question. We need to look at why the baby is breathing this way-is the tube position ok, any secretions, are we on the right mode and settings appropriate to the pathology? Constantly review the situation and don’t hesitate to change a mode that isn’t working. Look at the delta p, if it’s very low, as often happens with VG, you could consider a period on SIMV with PS with consistent pressure delivery. Excessive sedation and paralysis hide the problem temporarily. Use it only if no other option

What is usual dose of morphine Sir?we generally used Fentanyl as analgesic in Nicu?
