Indeed, Kishimoto straight up said "I don't know how Madara will be defeated. I made him too strong". He even went into writer's block just to think of some legit way of beating him. He couldn't since Madara was obviously too op to be taken down properly, and thus, that's where Black Zetsu proceeded to literally backstab Madara, and in came Kaguya. Kishimoto had to resort to a damn cop out just to "defeat" Madara. How many times has that actually happened in Anime/Manga history?

Not to mention, Isshiki was basically crumbling at that point, so maybe his perception was already fucked up at that point. Not so easy to sense a difference in someone's chakra (clone or not) when your entire being is basically becoming dust, I imagine.

Second closest is in one of the games, he has a so far non-canon ability to reverse time so he can avoid certain outcomes (Game is Storm Generations iirc). I always thought it had something to do with his Susano'o if he actually has any, would explain why his is so malleable (such as having four arms and multiple faces [Double Meteor scene] at times). Of all the Susano'o we've seen designs of, Only two other Susano'o share even a fraction of those features, being Sasuke and Itachi's, which occasionally have an additional forearm and hand (Itachi's holds the Gourd, Sasuke's generates arrows). Another thing I just recently remembered, Susano'os are supposed to all have more than five fingers. Clearest example is when we first see Sasuke's in the manga fully form, it has Six Fingers. They forgot this in the anime, and changed every Susano'o to have 5 fingers instead.
