Thanks for watching! I'm not an OET examiner so I can't know for certain the answer to your question. From my perspective I think letters are better if they are at least partly in active voice. However you can still use passive voice if it is appropriate for the meaning you need to convey. I hope that makes sense!

I would end: Yours sincerely Doctor The OET is marked on your candidate number and not your name so you shouldn't sign your name. In real life you would sign: Yours sincerely Dr Sourav Das (also sincerely should be lower case, I was doing it wrong my whole life, as do many other professional people, until someone commented here and I realised I made the mistake in the video 🤦)
Unless it was directly relevant to the timeline I would include medication in the "background" section. For most circumstances it would be appropriate to write current medication with past history.
If you are using my technique then there is not really a category of "present history". This would be "timeline" (otherwise known as "history of presenting complaint"). If the current medication is for an unrelated condition to the presenting complaint then this would fit better with the past history.
Regarding semi-colons, I do plan to make a video about them for the OET, but if you put "semi-colon list" into google/youtube there are a lot of great resources already!

This is a really good suggestion! I will try to work on some examples. In the mean time, in my opinion some repetition is inevitable between the introduction and conclusion, so I wouldn't worry about it too much :)

That is a very good question! I am also an English teacher so this is the exact type of question this channel is designed for! The short answer is the "a" is optional. It is correct with or without "a" The long answer is that "assessment" can be a countable OR a non-countable noun. The meaning is only slightly changed between the 2. In the countable sense you refer to 1 specific assessment by 1 specialist. In the uncountable sense you refer to assessment in general, that might involve a few appointments with different specialists (but might still be one). In most oet style letters both apply. Also, it comes down to common usage. In real life I find most referrals by native speakers use the uncountable form of assessment. However very few native speakers realise that there are 2 grammatical meanings of the word! I only learned this while studying for my English teaching qualification many years after qualifying as a doctor!

We use "transfer" if you are physically moving the patient at that time. This would be common in the nursing exam if you are writing a letter for when a patient is transferred from a hospital to a nursing home for example. Additionally you can write "I am writing to transfer this patient back into your care" if the context is a specialist handing a patient back to the GP
Am not comfortable with the introduction saying I am writing to refer this patient without stating their names it’s rude and it’s like they’re just an object noooooo! I would think we replace that statement with, I am writing to refer Mary Tanned, 29years old patient with symptoms then you continue.

The correct form is actually "Yours sincerely" with NO capital. I only learned that I had been doing it wrong my whole life AFTER I made this video!

Mostly I have seen they ask for referral letters from a General Practitioner to any specialist who is also a doctor.
Is this only for Doctors or nurses too?
