      I've watched quite a few episodes of "Mayday," and 99 percent of the air crashes occur because multiple systemic mistakes are made that cause -- and contribute to -- the crash. It's rare that only one factor is the cause.

    I am a private pilot.
    The Learjet plunged at a steep angle to the point of impact. It appeared to be on fire. There was a fireball upon impact.
    Photos of the accident scene show a large section of the fuselage that includes an emergency exit laying in the street.
    This section of the fuselage has the words "EMERGENCY EXIT" in English and Spanish. This portion of the fuselage has two windows. One is completely blown out and the one in the emergency exit is partially blown out and damaged.
    There is no fire damage to the exterior portion of this fuselage section. This portion of the fuselage appears to have been catastrophically broken away from the fuselage prior to impact and is deformed outward as if there was an interior explosion inside the aircraft.
    The interior fuselage structure shows charring on the interior portion of aluminum fuselage. The force of the explosion (in my opinion) caused a ripping of the aluminum at the joint line (see photo).

    Obviously something catastrophic. My cousin died landing on a runway in a small plane at a private airport. Complete engine failure caused the plane to flip at the last second. He had been flying for almost 50 years with over 3 decades of being an instructor. He has 10s of thousands of flying hours.These things can happen to even the most experienced. It's so sad, prayers to all involved!

    How is just one ATC in any tower acceptable ever? That should never be the case. At any moment anywhere ATC has a medical issue, passes out and all aircraft are left open to disaster!

      They are going to try and pin this on the helicopter crew and the ATC when it was really the system at large that was at fault. We all know how higher ups will try to cover there asses.

      IMO the Philly crash is not ATC related. From the trajectory, the pilot must have been pushing the plane _into_ the dive... it's a parabolic arc _down._

      @@rickr530Former instructor here. I don't believe it stalled -- it certainly was not stalled when it hit. It was at a significant speed, following a smooth, arcing flight path and the fact that the aircraft was able to change its flight path either due to aerodynamic forces or control inputs in the last second indicates an airplane that was flying. Watch the pitch change in the last second. There was an LOC (loss of control) situation, but it looked to me like the airplane was flying. Possible causes could include hydraulic failure, flight control system malfunction, pilot disorientation, uncontained engine failure leading to tail or control surface damage, and so on. A stall or stall/spin covers a limited horizontal distance over the ground and does not generally proscribe a smooth ballistic arc.

    30 seconds into the flight they obviously had a major mechanical issue.
    Possibly a fuel leak.

    That was a perfect interview! It was quick, informative, there was an early acknowledgement that this discussion will be speculative in nature and they assumed their audience was reasonably intelligent. I feel like this was representative of what most major news channels' interviews were like in the 1990's when I was growing up.

      I was thinking the same thing! I like this reporter. He asked genuine questions without trying to pressure the guest into crossing the line from speculation to assumption.
      In return for his respectful interview, the pilot gave us all a unique perspective when he explained how jets go full throttle when starting as opposed to larger aircraft.

    I am an Army brat and my RIP Dad was stationed at Ft BELVOIR and I have been to there many times and watch the Helicopters over the Potomac and my house EVERY DAY...from Davison Airfield.. I grew up here. There should be NO HELICOPTERS PRACTICING AT DCA. I worked at Andrews AFB and I never saw Helicopters practicing there. Also WHY IS THE THIRD HELO PILOT NOT BEING REVEALED?

      @@MasterCaine That would be unlikely. The engines have shields because of birds and weather ballons ect. In any case that something _did_ make its way in there, it wouldn't cause the plane to ignite like that, and would still be able to make an emergency landing using the other engine. Depending on the model of plane and how many engines it has, it would even be still capable of flying with the loss of an engine.

    I'm going to take a totally unsubstantiated guess at what caused this.
    Being a medical flight, with two doctors, a patient, and a family member on board, in addition to the two pilots, I am going to assume oxygen was involved, and something caused the oxygen cylinder to rapidly leak, igniting the atmosphere in the fuselage, which would explain the flames seen, and an aircraft completely devoid of control from either pilot.
    If so, that was a horrible few seconds before impact.

      It was done on purpose. Too many coincidences. Chopper was not on course, flying higher than allowed as well. They were warned 2 times and had a straight shot at the plane. No way they couldn't see it. Plane tried turning away and chopper turned to make sure it hit.

    Plane crashes happen multiple times a day in the US, but a "crash" has WIDE definition from running off the runway to catastrophic I've been in a "crash" when our private plane broke an oil line and we had to land in a farmer's field with hay bales.
    These days if there's no footage, witnesses, or loss of life it doesn't make the news and we never hear about it.
