  • Last February, Abi Duyile’s 13-year-old daughter, Hannah, suffered a fatal allergic reaction after sipping a supposedly dairy-free hot chocolate at Costa. Despite seeking help, the only available EpiPen was a third of her prescribed dosage. An inquest blamed Costa staff's failure to follow allergy protocols, but Abi has never received an apology. Abi and her lawyer, Miranda, share their story to prevent similar tragedies.

    Mum is so strong and well composed. The way she is talking about her daughter in past sense is heartbreaking.

    As the mum of a son who has life-threatening allergies, I find this heartbreaking and terrifying in equal measure. My heart goes out to this lady and her family. Absolutely tragic.

      I have some allergy training, and I don’t think it’s expressed enough just how dangerous milk allergies can be. You hear about nut allergies and allergies to other things that can be really serious, but milk allergies aren’t automatically *up there* with the most serious cases. I will definitely be taking dairy allergies a LOT more seriously from now on.

    As someone who worked in hospitality I was always SO vigilant when it came to allergies. You can never be too careful. I really believe hospitality staff need to be trained more on this as it is so important and can cost a life as we see here. Just awful.

      I’m not in hospitality but once at work a coworker practically made fun of me for being nervous about cross contamination knowing that we had people with allergies. People don’t take it seriously.

      I agree, I've always worked in hospitality, and I was a manager of a Costa for a few years.
      Costa is the the most demanding company I've ever worked for, when it comes to company standards.
      But I refused to employ students after being there a while. They don't care about anything and will always cause you more work. The amount of times I caught people cutting a vegetarian sandwich with the bacon knife. Putting full caffeine coffee in when people had asked for low caffeine.
      You always do food quality when in hospitality, but I never remember learning about allergies(except for allergies to plasters).

      I'm in hospitality. My place of work has a rule where we always ask customers if they have an allergy or if it's just a preference when they ask us to take something out.

      That's a fantastic idea...i have some really bad allergies and always feel like an "annoying customer" asking them to double and triple check what they have used.

      That is a really good idea to possibly avoid such a tragedy like this in the future, but I hope you do realise that by doing this option to ensure your own personal safety it completely eradicates any element of trust you're meant to have in your fellow man, which is really sad by the way😥
      I don't believe Hannah and Abi were perhaps as reckless as some may think as both of them would of had to have acknowledged that there was still a level of risk in deciding to eat out, even if they truly thought it was quite low as they were just grabbing cups of hot chocolate☕

      Her epipen should be on her at all times tho. Unfortunately if you have such severe allergies you have to be so careful.

My mum would stress to my brother to always carry his EpiPen, even though he's never had a bad reaction to something he eaten. (He still doesn't carry) But If it did happen, I kid U not , the 1st thing my mums gonna ask him where his EpiPen was and give him a lecture

      I know that people today don’t seem to get this, but it’s entirely possible to hold two emotions at once: 1) that this is a tragedy and 2) that this was largely preventable.

      Epi pens have 2 colours, one end is orange and the other is blue!
      I
      The blue end is the safety cap which you remove before administering and then you firmly press the orange end in the thigh!
      And then you must go to hospital still!

    My issue here is why hasn’t costa apologised? I ABSOLUTELY agree that some people working at Costa have a rude attitude when it comes to ordering , this is ABSURD that they have emotional people working there!
    Costa needs to apologise and she needs to SUE! Its unfair especially i feel like some people serving at costa don’t have a conscientious way of working because if they did, they wouldn’t have made that mistake! Allergies are sudden and random!

      I totally agree, Costa need to address this but this was a franchise store. The actual Owner of the store or the company that owns the store needs to apologise. Costa (owned by Coca-Cola) is a different business. In my area, most of the Costas are owned by a company called scoffs, im sure this will be a different franchisee, not scoffs, but they need to be prosecuted and answer the questions as they are in charge of staff training and liable for this

    I'm not trying to be rude in any way but I seriously cannot believe that they did not already carry at least one epi-pen when she has such severe reactions. I'm sorry this happened...this woman's worst nightmare came true and it's just awful. But how could you not have an epi-pen already??? Why would you put yourself in a position to have to drive to the nearest pharmacy and hope that they have one? I can't wrap my head around that. RIP to that poor baby.

      I think they are based in the UK and it’s different over there. I read a comment that said over there, you don’t have it prescribed to you until you have a severe episode also just from the pharmacy only having so much also indicates things are not the same as in the States. I’m assuming you are speaking from here as well.

    Labelling the drinks won’t stop server errors. I always ask for decaf coffee but have been served caffeinated coffee before, despite their being a label on the cup.

      In Starbucks I always get decaffeinated, They manage to do it.
      I know it’s decaffeinated because I don’t get a reaction from it, I feel perfectly fine after drinking it.
      They also manage with my soy milk thankfully.

    I had a few people with allergies come into our restaurant, they want us to check the boxes that we get delivered, for their specific allergen. I had to tell a woman who told me “are you 100% sure there is no wheat on this, I’ll get a severe reaction” this after asking 5 diff people and going on the ingredient list of company who delivered to us, I got so nervous that I told her “look I cannot 100 percent garantee that there is no wheat added to this clam chowder I would not put my life in the hands of others because there are always errors with ingredients, there is none listed on the box, and 5 other employees said no wheat, but I would not take a chance if I were you” it is so scary that even a little drop can cause this, every time I was asked I would say that I cannot and will not garantee.
    This is so tragic I’m so so sorry that this happened

      Amen! I was the same when I worked in food service. We were not a nut free, dairy free whatever else free establishment. My boss always told employees to make that very clear to customers with allergies. We would do our best but could make no guarantees.

    This was my worst fear when I worked in a coffee shop! We had a few regulars with severe allergies, and it opened my eyes to how difficult it is to avoid cross contamination and mistakes in that environment. Even when your barista is being careful, it is SO easy for something seemingly small to go wrong and cause big problems. When you have dozens of drinks on the line, you're using the same wands and jugs for all of them, customers grab the wrong drink or the barista misunderstands the writing on the cup - it is simply not a safe environment for severe allergies. And maybe this is a US thing but a small business could never afford to keep epipens on hand. I've actually never heard of that in my life.

      @leeg7625 I do remember hearing something semi recently about them lowering the price a bit so that number may be outdated, but epipens without a prescription are super pricey. I remember we looked into getting one for the cafe i worked at several years ago and one was like 700 bucks. I think all food establishments should have them on hand but I get why that's just not a thing you see here.

    The level of victim blaming in the comment section is bonkers. An investigation confirmed that the proper procedures were not followed. Due to negligence, a child lost their life, and the mother deserves to be compensated. Companies only seem to take notice when their finances are affected, which is why, during the peak of cancel culture, businesses began to prioritize their actions, lest they face closure. Period.

      That's the problem - ive worked as a chef for past 20 years. People like telling you they are allergic to anything they dont like. Some we take seriously, most of them, we don't. Sorry not sorry - that's the reality.
      I work in the seafood restaurant now and I had people coming over to be allergic to oysters and their partners ordered oysters. Or people allergic to tomato, but they wanted tomato relish, they just didnt like sliced tomatoes.
      My point is - if someone is SO freaking allergic to anything, as a mother, i would NEVER take a chance to put my kid in risk. Just use your brain - do you seriously think people will follow your demends? No! Not in a busy place like Costa, so why would you even try to go there, if you know that your kid is so highly allergic to most things they have there?
      It's a huge tragedy, but it could have been prevented.

      It's not that someone does things on purpose, but under pressure of time - its easy to make a mistake. That's why if you have a real, severe allergy - you should really avoid eating or drinking out. Or always has your EpiPen ready.

    A coffe shop would be completely off the list..esp when u can easily make this stuff at home.. .my granddaughter is allergic to shellfish so that means nobody can eat seafood if shes with us...period.

Yes, there are places that cater, but as the other comment said, they mess up, and it only takes that ONE time to mess up, and destroy someone's life forever.

    I am a parent of a child with life threatening allergies. It took me along time to get over the constant anxiety but u have to let them live. I still get it as even i getting scared those who know his allergy will sabotage him in some way, for a joke, for anything, all sorts go thru ur mind. That said we have to rely alot on others n that leaves alot to be desired. Its very important we stress the allergy our child has. What i dnt understand is why she didnt have her epi pens. My child religiously carries his everywhere n he needs two epi pens. Any help that aids allergies is welcomed. My son uses restaurants that uses sepwrate kicthens for allergies. Ambiguous restaurants like costa my son wouldnt eat in. He panics alot too due to his condition. So sad for this girl n family ❤🙏

      @Fredrica2.0 I'm not quite understanding as epipens are issued to those who have anaphylaxis, if there was a shortage they can order from another area thru a docter, why did she need to go to a chemist? Was she not issued epipens? Was it clear she had anaphylaxis? Did the mother follow up prior with medical care being the normal route is specialist allergy clinic...or was this a sudden anaphylaxis attack?

      They got hot chocolate before the dentist. They reached the dentist where she started exhibiting symptoms and was offered an epipen from the dentist and the mother refused. Instead, she decided to go to the pharmacy and was given anthistamines 😒
      The pharmacy only had children dose epipens.

      I so agree. I've a severe nut allergy. I carry both epipens everywhere. Can't even count how many times I've even been given salad containing nuts. So sorry momma heartbreaking.

    Shame on the person who prepared their order, but mistakes and carelessness do happen. If you gave severe allergies you probably shouldn’t be patroning restaurants that are not allergen free. You can’t expect someone else to be responsible for your health. We have to accept that we cannot be entirely inclusive. Horrific tragedy.

      Allergies are becoming more common as it stands. So what's the plan-ban those with allergies from coffee shops altogether? Maybe go the extra mile and forbid them from going on holiday, too? No.The responsibility is on the eatery. I used to work in food service, and let me tell you, I didn’t have a clue about allergens. Instead of brushing off 'careless mistakes,' how about training staff properly and taking allergies seriously?

    With everyone talking about epipens, you're often not prescribed one until *after* a first anaphylactic shock (this happened to me). This may be why they did not have any.

      They had her tested at two where it was revealed she had many allergy’s including cows milk. Nonetheless this is tragic with so many allergies any meds to assist during an attack is mandatory and super vigilance when ordering..Hard to fully monitor when ordering in places where you are not in control. That’s why the epi pen or whatever else she took during an attack needed to be with her at all times. Also ambulance should have been called immediately.May the young lady R.I.P..

      I'm not blaming her at all but she did say that the pharmacy where they got the medication that they first gave to her daughter that didn't help with reaction, is the same pharmacy that provides them with epipens normally so she was definitely prescribed one. I guess for some reason unfortunately mum did not have one on her at the time, and neither did the pharmacy. Well, not one with the correct dosage anyway.

      They didn't have pens with them but where definitely prescribed as she new the dose of the medicine that she didn't have, hence why the pharmacist meds did not work as it was a lesser amount no one is wrong in saying if your given meds for somthing so serious you should be carrying them with you

    My 3 year old has a dairy allergy (thankfully not life threatening) but she’s been given dairy at least 3 times at cafes. It’s awful. People don’t take it seriously!

      My child has a milk allergy too. I wouldn't trust any barista/server to use an alternative milk, even if I asked for it. The likelihood that a person might make a mistake is just too high.

      Yeah I get that 100%! I have a cousin who has a mild allergy to peanuts and whenever I’m out with her and I literally say that she has an allergy to peanuts but then she says “no it’s fine. It’s not that bad” but I tell her that even if it’s not that severe or even just mild, it can still give you a reaction and it could get worse at any time so you always have to be careful.

    Oh my god this is just so tragic, not only is it absolutely horrible that the Costa worker didn’t make the order correct especially after being told her daughter has a severe allergy, but the fact the pharmacy didn’t have enough dosage as well is just sad. She just kept getting let down by people. Im so so sorry for her loss, may Hannah rest in peace she was taken way too soon. I truly hope action gets taken and that this doesn’t happen again.       Yeah that caught me as being really bad also because as well as Costa being held to account for their part in this poor girl's death, that pharmacy should definitely be investigated as well. I understand a pharmacy can't be able to deal with something as serious as a gunshot wound for instance, but they definitely should have the necessary supplies for people who have serious conditions or illnesses like diabetes medicine and the full dose of AAI's for allergies all on hand.
      So I really do hope they're looked into and if it's found they should have been able to have done a lot more for Hannah then there should be penalties imposed for failing to help her properly.

    I wouldn't usually comment but I think its important.. I used to work for Costa Coffee here in Ireland and it had to be one of the most stressful environments I was ever in. The company do not care of the well being of staff members so that poor guy/girl that served was obviously not trained in properly! They employ people that do not want to learn about the importance of these things it took six months before I was trained properly on coffees and that was in a different store not even by my own manager! It has taking for something like this for Costa Coffee to own up to their responsibility! We were told to aim at the kids for the hot chocolates ask do they want marshmallows and cream to rob the poor kids more! Ugh Smh!!

    Costa is to blame for giving her the drink but the mom made many fatal mistakes too that contributed. You should have immediately called the Ambulance for your daughter. And even when the pharmacy ASKED you if they should call the ambulance you said NO. But before all that you should have had the epipens with you! At all times! The time you spent going to the pharmacy, you could have administered the pen and driven to the hospital to make sure it helped her. In life we have choices and unfortunately you made many of the wrong ones.

      People panic in such situations, that’s why they say family members aren’t the best people to make decisions with their loved ones care because they are not thinking rationally ,I’m medically trained but lemme tell you something when my niece had a temperature and her mum was panicking all my medical knowledge left me it’s like I just froze all I could do was hug her . I lost one of my friends due to a similar situation , her family made choices that wouldn’t have been made if they were treating someone else . I can’t explain it but it’s like we are blinded because of that emotional attachment 💔

      Costas fault too, they should give up to date thorough training to all their staff with an emphasis on allergens and food safety. The girl that served them didn’t seem to take it so seriously so the training wasn’t sufficient. The average person probably doesn’t know how serious some allergies can be.

Most of us know a person can die from an allergic reaction. But some people think it’s rarer than it is and a lot of people for some reason believe the odds are always in their own favor. Sure, there are people who can die from drinking milk but surely the customer who told them they’re allergic to milk can’t be one of those people. That is how a lot of people think.

      No shops shouldn't have an epipen like they can't have a glucagon injection (for diabetics hypos) as they're NOT allowed to have these in a first aid kit. Not all stores have trained first aiders working. Not just that it's classed as ABH

      No. Even a pharmacy can’t give an epi pen over without a prescription here in Ireland and that resulted in a death a few years ago.
      As a parent of a child with an allergy, you carry two epi pens with you at all times - one as a back up in case it fails.
      In school they have two. In creche they have two.

    This happened to me in Costa too. I asked for non dairy milk and they put milk in it. I'm sorry but it's not be fussy at all. It's called having empathy. It takes two seconds to double check the milk, clean a jug and turn steamer on.

    All Dental practices have epi pens and are trained to deal with allergic reactions. She should have told a member of staff in the dentist and they would have a duty to administer care! Epi pen used and ambulance called immediately!

      I read in another post that the dentist offered an epi pen but the mother declined to use it. She left her daughter in the dentist by herself while she went somewhere else, that’s why the daughter called her mobile: mother had to come back then they went to the pharmacy.

    Costa does this all the time, my sister ordered a vegan pumpkin spiced latte as she’s intolerant to milk and they still put cows milk in it. The barrister said it’s classed a vegan as the syrup doesn’t have milk in it … but they still added cows milk?! If you’re allergic, intolerant or a vegan, keep an eye on your drink when they make it as they’ll put any old milk in it regardless of what you ask.

    I'm not sure about Costa, but most of the hot chocolate powders they use in coffee shops already contain milk. I'm also allergic to milk and I avoid hot chocolate for this reason. in the case of this girl a mistake was made with the actual milk used, but I wanted to specify.

      although i am vegan and i asked to look at the allergy sheet as the woman was unsure it says the hot chocolate has powdered milk in it so I'm very confused about how it would have been okay to have it regardless they may have used the right milk, i think its poor advertising on Costas behalf saying that u can have a hot chocolate with milk alternatives but it still having milk powder in the hot chocolate powder.

    As a family with allergies and intolerances, this is absolutely our worst nightmare. NEVER trust someone else with your life, ALWAYS carry your epipen if you are prescribed one. Personally, as i only have a dairy intolerance and not an allergy, i do sometimes have a soya drink from costa. I ALWAYS WATCH THEM put the correct milk in, twice ive caught them using the wrong milk. Will some food and drink establishments learn from this? Sadly not, because they will employ staff that dont give a toss. I have questions..do dental practices not hold drugs similar to epipen? Did this family not have theirs with them? Why would you risk getting food or beverage with such a plethora or allergies, if you dont have your epipen on you? Surely the cross contamination alone would induce a reaction for someone with an allergy so severe? I know we all want to enjoy what everyone else does, i get it, ive been there, but us it really worth the risk? Tragic, avoidable death, poor child.

      It doesn’t make sense tho as she said in the video that her daughter also had a severe allergy to some nuts etc… as soon as your child has a reaction to any food the nhs are pretty good at testing and giving out the correct medication. She also said that she went to the pharmacy that she uses to collect her daughters allergy medicine? And they had it and home so I’m unsure why she would carry this?
      I carry around a bottle of antihistamines and 2 epipens. My daughter did not have to have a bad reaction either to get her epipens at the hospital… they could tell just by tearing.

    This is so sad. My son has life threatening allergies but he carries his own emergency kit with EpiPens in everywhere he goes. I am surprised this was not the case here, however if she had not had a previous anaphylactic reaction her dr may not have thought she needed it. People without allergies find it hard to believe that food causes death in some poor people but allergies are on the rise and are very serious 😢

      And that's another thing that I thought of when I was hearing this as well. I've got no idea why that was as it could have been carried very easily in one of their bags but I guess it might be about living free from your health condition and not letting it weigh you down, and so then to bring a AAI would just remind Hannah that she had a serious health condition for life. Also they may have decided to have trust that the hot chocolates they ordered would have been prepared the way that they had requested. But you've got to hope for the best, but l plan for the worst.

    I am having a hard time understanding why they didn't have an EpiPen on hand at all times and why they allowed their child to eat outside the home! I am on a special diet and I haven't eaten out in years nor do I eat at other people's homes! Did they not have the EpiPen because the manufacturer has drastically overprice the medication, it is expensive? I am sorry for her loss!

    Whew, God help her. I’m anaphylactic and this is why I do not trust eating out or even grabbing a drink in public unless I get it out of the fridge myself. People don’t pay attention or they down play my allergies bc they’ve never experienced for themselves. May God help her day to day. 🙏🏾

    I have intolerances and the number of mistakes I’ve encountered is shocking, awareness is so patchy, if I had a true allergy I don’t think I’d ever eat out, it’s too risky. Staff often take your request as ‘fussiness’ or a choice, so don’t take it serious, cross contamination is a thing when they are busy. I can see how there could so easily be mistakes with coffee in a busy place where there is a line of drinks being made. Best stick to small coffee shops where you can get individual attention and make sure you have definitely been heard. A sticker or flag on the food or drink would help as it does give you the reassurance that they have heard you. Always have epipen with you if you have these issues too as you never know what could happen. Sadly too many have already lost their life like this

    My heart goes out to this Mama, I only read the description and watched a few seconds and it's overwhelming. Costa are not accepting culpability due to the legal ramifications and I expect also carrying out an internal investigation. In a case such as this I would have hoped ethics, having a moral compass and taking accountability regardless of an investigation, reputational damage and financial repercussions would have had more currency. I have never heard of someone having such a severe reaction to dairy, I think as a society we should have more information, potentially this leaves an unknown percentage at risk and a society that is uninformed, with businesses needing to prioritise the implementation of mitigations.

      Have you ever looked into treatments for the allergies?
      It costs money and I don't know the cut off point, but if I were in your shoes I would try to do everything in my power to make life easier in that regard. Depending on the situation, I believe this treatment should be available on the NHS, it would most likely save money in the long run..

    If your child is this sick/allergic, and you choose to take a risk by trusting and ordering a drink made by a teen or someone who barely speaks English at a cafe, you should AT LEAST have both* Benadryl and an Epipen of right dosage ON YOU because you are taking the risk in ordering out! I’m so sorry this happened but there were things the mother could have done as well, instead of trying to run to a pharmacy in a panic and hoping they have the right dosage. It’s the mom’s negligence as well.

      Realistically more than the majority of the staff/job applicants are never gonna be able to get it right or really apply what they've learned in the test into active duty serving customers that do have serious health conditions. And all Costa and all other companies are doing is trying to transform the role of a barista into some sort of healthcare assistant and that's not what these people signed up for. So for all the ones that are managing to excel through the test, the very first time, the things they've learned still may not stick as it's not what they're cut out for period.

    This is so sad! Please don’t blame the mum, it’s not her fault😭😭💔 - My little cousin has the same allergy and it’s so scary😭 and My other cousin who is also called Hannah🥹 is a few a days older than me, not long was her birthday when she passed away last year December from peanut allergy, (eating a kebab) - She always made sure that her allergy was mentioned before ordering food from shops! The Epi pen didn’t work and along with that not sure if it was a heart attack or asthma attack! But it led her to be put in an induced coma! I find out the day before my birthday and then had to visit my cousin in ICU the day after my birthday to say goodbye! Nothing could be done 😭💔

    I worked in hospitality for 15 years. And its still only online training for severe allergys. I think every restaurant, cafe etc should have epi pens in the medical box and staff trained to administer the pen and also extensive training for allergys, in the same way hospital staff would be trained.
    The amount of times in my time in hospitality I have seen mistakes happen which could cause a severe reaction to someone with allergys is unbelievable.

      Because as the mum says, she took a Cetirizine, which usually helps.
      The Cetirizine is like an epi-pen for me. It helps with my allergies, I’ve never thought it could get worse than usual, but now I’m going to get an epi-pen because it’s not worth the risk.
      I never thought I’d need an epi-pen, like this family, but hearing how her’s got really bad really quickly, I can see how it could happen.

      If someone is prescribed an epipen you are meant to carry around with you incase you need it. if you have diabetes, what use is having insulin at home and you're out at nandos.

      'Not victim blaming but'.. Because she'd never had a severe reaction before, all of her previous reactions had been relatively mild and were managed with antihistamines. She's coming on national TV to raise awareness of this to hopefully prevent more deaths. I'm sure the mother thinks about this every day of her life, no need to kick her whilst she is down. She's trying to raise awareness to help other people!

    I have a son with severe allergic reactions to pollen, dust , and nuts. Every March he's gets so sick on me. When he gets a allergic reaction he screams 😢. I even have ended up in th e.r. My son never goes out in the summer and even winter can be bad cause he has eczema. Sometimes it's hard to tell what's safe to do and how to fix it. My heart goes out to this mom, she did all she could.

      I think it's because she was given cows milk not soy milk as they usually order this drink and are fine. The point was they were given the wrong drink to what was asked for and Costa agrees they were wrong.
      *Again just want to make clear they seem to be fiine with coco powder that contains milk (as this is their usual drink) but they replace the soy with cows milk as thats what sets off her allergy

      Also, to clean the machine of all milk traces, that machine needs taking to pieces.
      The wand (steamer) will have milk settled in the spout. Opening it up a bit won't clear it completely. You need to take it apart and clean all the small parts by hand.

    One mistake was not immediately taking her to the hospital. Why “wait a minute.” You go to the ER immediately. As soon as I think I consumed something I am allergic to, I am going straight to the ER-Epi or not. When you have a deadly allergy, there is no waiting for anything. You immediately call 911. I’m sorry mother. That waiting was not the way to go. Sorry.
    Overall, that Coffee Shop should be held accountable for having an inefficient employee.

    I usually wouldn't comment but hearing the mother say "she wonders if there is something she could have done" maybe she wont read these comments but honestly the mother seems incredibly strong and clearly has taken of her daughter so incredibly well that she has never had a severe reaction. Your kid having that many allergies and STILL the mother ensured the worker knew the importance of washing out the jugs but the server was the one who didn't seem to bother. Completly Costa's fault, server should be sued and fired. This is just not acceptable at all.

    People who have such severe allergies SHOULD NOT patron at these places. I would NEVER put my life into the hands of an uneducated employee- most of them don't take allergies seriously, most of them could care less about what you suffer from and the importance of using a clean mug to mix drinks therefore, if my allergic reaction is life-threatening I would never trust them to get it right because they are NOT trained sufficiently. Mom, WHY were you not equipped with an EpiPen?? It seems that she became "comfortable." A good lawyer will eat this case up in defence of the establishment and win!
