EM doc here do a lot of POCUS
SO what im taking is to first zoom out and look for any segments getting pulled along by others, so to speak. then to look carefully through each piece of the wall and look for akinesis, followed by the same for any thickening of the myocardium. any bit that isnt thickening or is thinning and bulging warrants attention. lastly look at the diameter of the ventricle to see if it is decreasing symmetrically to reflect normal movement, and if it is staying the same look more closely for a/hypokintic segments. last step look at surrounding structures, valves, tamponade, fat pad, etc.
sound more or less right for a simple old er doc with a US?
