Did you install number 1 jaw to the number 1 slot? Did you try tightening from all three nuts, only one should be true. back in the old days(when I was still in Cluj) we used to use a die at the last set of teeth and grind until then, and at the end we ground off those last set of teeth. Also another thing that could be done, is to make an adjustable chuck, with set screws on the side of the chuck and increase the gap between the chuck and the spindle step. These are called Set-true chucks.

Chuck should be rotating as slow as possible while grinding
