She is Alune the twin sister of Aphelios, she is in another plane, to communicate with her Aphelios takes a strong poison that prevents him from speaking (In the game the one who speaks is Alune no Aphelios)
She is also the one who sends the weapons to Aphelios: 3

Yup,the only time Aphelios ever vocalizes is when he's in pain at low HP, or right after respawning.
I love how, since Aphelios is mute and cant respond through voice, their simbiotic relationship of one needing the other (cause of the state they are and fate and powers of the moons, yeah plural) you can take the instruments as Aphelios, you can see how the more the voice goes the more the intruments show how he feels, his readiness to fight for what he believes, and also since he got a bunch of weapons bunch of instruments.s
So Aphelios can actually speak, but the pain of the poison causes so much pain that he actually can't, that's why you can hear him yell with his ultimate.
Aphelios and Alune are twins born to the Lunari tribe on mount Targon. They value faith very much and worship the moon. In an event the Solari ambushed the Lunari just as the two moons converged. Meaning that the moon in physical realm and the moon in spirit realm was in the same place at the same time. The spirit temple Marus Omegnum which only accepts one sorcerer inside at a time chose Alune and Alune saved the temple by pushing it back in to the spirit realm while she was still inside. Aphelios now drinks the poison of the Noctum flower which renders him mute and gives him so much pain he gets numb to the other sensations. With the poison Alune is communicating to him from the temple and sending him different magical moonstone weapons for him to use. Together they protect the faith of Lunari.

*Ehem I'm Aphelios's lover* I mean that's Alune. Aphelios's sister. No cap, her body is stuck in the moon temple and her soul goes with Aphelios and Alune provides the guns. Alune is the one talking because Aphelios is mute. They are actually Lunari like the girl in Tales of Runeterra: The vaulted road, the one who was shot by the arrow and Diana. So these Lunari people, Aphelios and Alune, are with Diana's movement, pushing the Lunari faith up there and out the shadows.
So Aphelios and Alune works is Alune summons God guns basically in the spirit realm to physically be held by Aphelios and use it as their weapons. You see only was allowed in the Moon temple that day they went there so since that was the case, Alune made it so that Aphelios is able to fight for them and her for them too in their own ways.

The girl is his twin sister, Alune. She gives him access to special moonstone weapons, granted to him from her in the spirit realm where she resides. In game once a weapon is out of ammo, Alune swaps it out for another like she's doing in the picture. She's taking the crescent looking one called (Severum) and giving him the sniper looking one (Calibrum) on his back. There are 5 in total that she cycles through.
You should totally react to the Aurelion Sol teaser trailer and his champion theme! A really awesome champion with great soundtrack. Glad you already reacted to aphelios btw~
