  • ✅What are the treatment options in Congenital Talipes Equinovarus (Clubfoot)?
    Because your newborn's bones, joints and tendons are very flexible, treatment for clubfoot usually begins in the first week or two after birth. The goal of treatment is to improve the way your child's foot looks and works before he or she learns to walk, in hopes of preventing long-term disabilities.
    Treatment options include-
    Stretching and casting (Ponseti method)
    Surgery
    Stretching and casting (Ponseti method)-
    This is the most common treatment for clubfoot. Your doctor will-
    Move your baby's foot into a correct position and then place it in a cast to hold it there
    Reposition and recast your baby's foot once a week for several months
    Perform a minor surgical procedure to lengthen the Achilles tendon (percutaneous Achilles tenotomy) toward the end of this process.
    After the shape of your baby's foot is realigned, you'll need to maintain it with one or more of the following:
    Doing stretching exercises with your baby
    Putting your child in special shoes and braces
    Making sure your child wears the shoes and braces as long as needed - usually full time for three months, and then at night and during naps for up to three years
    For this method to be successful, you'll need to apply the braces according to your doctor's directions so that the foot doesn't return to its original position. The main reason this procedure sometimes doesn't work is because the braces are not used as directed.
    ✅Surgery Congenital Talipes Equinovarus (Clubfoot)-
    If your baby's clubfoot is severe or doesn't respond to nonsurgical treatments, more-invasive surgery may be needed. An orthopedic surgeon can lengthen or reposition tendons and ligaments to help ease the foot into a better position. After surgery, your child will be in a cast for up to two months, and then need to wear a brace for a year or so to prevent the clubfoot from coming back.
    Even with treatment, clubfoot may not be totally correctable. But in most cases, babies who are treated early grow up to wear ordinary shoes and lead full, active lives.
    ✅Physiotherapy in Congenital Talipes Equinovarus (Clubfoot)
    ⭕Massaging the Foot -
    After the cast is removed, your baby’s foot might be sensitive to touch. During the hour that the child spends out of the boots and bar, you should gently massage the foot with lotion or cream. Use different strokes (e.g., tapping, tickling) and surfaces (e.g., cotton balls, wash cloths) to rub the foot. This will help desensitize the foot.
    ⭕Stretching Into Abduction and Dorsiflexion (Out and Up) -
    Stretch your baby’s foot out (i.e., abduction) and up (i.e., dorsiflexion) when your child spends time out of the boots and bar. Think about how the last cast looked, and try to stretch the foot into the same position. Hold the stretch for 20 seconds and repeat five times.
    ⭕Stretching Into Plantar Flexion (Down)-
    To stretch your child’s ankle, place one thumb across your child’s lower shin (just above the ankle joint) and the other thumb across the midfoot. Push down on your child’s foot to stretch it into a toe-down posture. Hold the stretch for 20 seconds and repeat five times.
    ⭕Stretching the Achilles Tendon-
    With your child’s knee straight, cup your child’s heel in the palm of your hand. Pull down on the heel as you use your other hand to push up on the midfoot (be careful not to push up on the toes). Hold the position for 20 seconds and repeat five times.
    ⭕Ankle Dorsiflexion-
    Starting position: Lay child on back . Place one hand on your baby's flexed knee. Grasp your baby's foot with the palm of your other hand placing your index finger above the heel. Now you can gently flex the ankle up and extend it down.
    Motion: Pull heel down and bend ankle as much as possible.
    ⭕Ankle Inversion and Eversion-
    Starting position: Lay child on back.
    Hand placement: Stabilize with one hand on leg just above the ankle.
    Motion: At the base of the big toe use other hand to turn foot in.
