  • We dissect the nuances of when to choose low flow (nasal cannula) and high flow (nonrebreather mask)
    Nasal cannula: 2-6 lpm
    Nonrebreather mask: 12-15 lpm

NRB is not a high-flow device. A true high flow device provides all of the patient's inspiratory flow, which creates a stable and fixed FiO2. NRB is a low-flow, variable FiO2 device because it does not provide the entire inspiratory flow a patient needs. What you set a flowrate to on a flow meter does not indicate whether a device is "low-flow" or "high-flow."

Your right I am a disabled
Paramedic , a Venti mask (Air entrainment mask) is a high flow device. they do have high flow nasal cannulas (7-15 lpm 48%-85% of o2 in the nose its very hard to actually reach 100% unless its via Trach or endotracheal tube, which I thought he was going to actual show but its not a high flow device; and i must note even the high flow nasal cannula is not a actual high flow device. a high flow device is actually supposed to deliver up to 60+ lpm to meet resp. demand. also they have a actual higher flow nasal cannula that delivers up to I think around 60~90 lpm.

Gotcha. that makes sense. We simply used that terminology to differentiate the ranges we would use with each respective device. I wouldn't increase the flow rate beyond 6lpm on NC. I've responded to patients with nose bleeds because their cannulas were on a high rate.

@@ChicagoEMTTraining yes I understand you should not use it with a general low flow or normal flow NC but we do have high flow nasal cannulas w/ high flow humidifiers. for 6.5 liters and above; all the way up to 15 liters. and it works pretty well if you need something that the patient could not handle the Non-rebreather mask or is claustrophobic.

@@christiansallee427 that makes more sense. We usually do not utilize humidifiers in the acute prehospital setting

@@ChicagoEMTTraining you might need to ask your Hosptial that you work with and your agency about high flow NC and high flow humidifiers. now you need the one that says 6-15 lpm which mine is white top.

I got nasal cannula with 15 litres O2 support due to severe Covid pneumonitis! Dangerous!

@@ChicagoEMTTraining Oh yes absolutely! My saturation level was below 88% and as soon as they put me on oxygen there was a sudden sense of relief. With time, it gradually reduced to 8ltrs, then 5 and finally 2ltrs!

@@akshatgulati7460 hope your getting better and you received your high flow NC w/ a high flow humidifier for 6-15 lpm so you didn't get nasal polyps.

@@christiansallee427 Thank you for your wishes. Yes I'm recovering. Also, yes it was a high flow humidifier! But they used to change the nasal cannula every alternate day to avoid secondary infections like polyps or mucormycosis! Severe Covid treatment involves use of high dose Steroids and other immunosuppressive IVs so they couldn't take the risk.

Hi. How long did it take to recover?? My husband is currently in the hospital with COVID pneumonia. He’s on heated high flow at 15lpm and his numbers go between high 80s to mid 90s Im just looking for a little bit of hope. We thought he’d be at 100 with it especially with 15 liters! I’m a freaking mess. Any words of advice are greatly appreciated.
