I think what triggered the alert (seemingly at random) is you went in, attacked someone and then left the room and then came back in again (causing it to autosave), so you kept reloading the save where someone was still reacting to being attacked.
Also, I'm sure you know but it deserves pointing out for future MGS excursions: boxes are more inconspicuous when they don't move 😜

Yeah everyone struggles the first time they play a metal gear game, for me the most difficult metal gear games are the originals from the msx
