Its an old Sustainiac.. you hook up your guitar to a pedal that you then wire back to the box on the head. On the pedal you can then choose what harmonics you want to feed back in to the neck of the guitar and how strong etc.. its what makes his leads absolutely scream on BFTA for instance(Ive talked to Neil Kernon about this.).
It connects to an original type of sustainiac pedal. A cable from the headstock connects to the pedal. The pedal causes some sort of vibration that is sympathetic to string vibrations causing them to sustain for an extended period of time.
I still have my Mighty Mite guitar body that his M1 is built from. And that black box is the magnetic coupler for a Sustaniac harmonic generator. I also still have mine lol.
either that or it just hangs from the headstock down to the pedal.. its not really something you would use live, its more for studio-situations(Live you would probably use real feedback instead.).

Something interesting of note....he mentions Karl Sandoval, a very famous luthier in SoCal. He did some work for this guy named Eddie Van Halen??? He also created Randy Rhoads' signature polka dot V by RR's specifications. Just another notch in rock and roll history.
randy got the idea for his polka dot v from George. George and randy were hanging out and George brought a flying v with a Strat trem that Karl Sandoval made for him and he told randy he should call him and have Sandoval make him a guitar and thats how the polka dot v was created

its a guitar he made from miscellaneous parts. he has had a dozen different necks and pick ups in it. he just has an esp neck because thats the brand that endorses him
