Not always- if the patient has sufficient adipose tissue, you can go in at 90 degrees. If thinner/less fatty tissue you go in at 45 degrees and release the skin fold, inject 😀- her needle is too short to reach any muscle
Literally what they told her to though. I think there are advantages to this, the way of holding makes the hand very free, and the speed through the skin should make it hurt less.

You do this when you do intramuscular
That’s for an IM injection and that is a very outdated practice. Aspiration is no longer recommended and hasn’t been for years.
You dont do that anymore
