    Dear ,there's couple of mistake done ... First you need to use Z tract technique , 2nd you don't aspirate , and #rd absolutely Not massage after injections are done.

    Aspiration is no longer recommended for IM injections, neither is the pinching technique. Z-Track technique is the preferred method.

      Absolutely right!
      And do not massage injection site otherwise medication will regurgitate back into subcutaneous tissue, that's not what we want from IM injection.

      As an instructor..aspiration is necessary as a safety precaution. I am teaching students. Aspiration is not necessary in the field, and my students understand that. This technique is for safety until they know they are injecting in the proper area.

      Z track is only used for meds irritating to the subcut tissue.

      Aspiration is necessary if you hit a vein it can cause heart problems.

      @@evelynsommerville In my nursing program they explicitly teach us not to aspirate for IM in the deltoid area. Have been giving vaccinations to the public- we also are taught not to aspirate there.

    Best to keep the needle covered with the safety clasp until you inject.
    Waving the exposed needle around is not advisable.
    Also, best to withdraw with one needle, and use a new needle to inject (puncturing the vial’s rubber membrane will dull the needle; an unused new needle is sharper, thus less painful).

    At last correct technique which includes aspiration. and for anyone saying not needed, it costs about 2 secs and can prevent injection into a blood vessel, rare but why take the risk with sloppy technique. I would insist on aspiration and refuse jab if not given.

      It has been deemed unnecessary because the US is driven by pharma companies that advice the health regulators. This means that to make vaccines more attractive it is deemed unnecessary. But vaccines are different then medications and are often less severe in harmful effect then a powerful drugm, it says on the individual package. But like the cuvishot you see the mess it can make when the vaccine flows into the bloodstream to the heart or brain, body reacts strongly and you get miocarditic, fatigue, brainfog and a weakened immune system. Because the body effectively thinks it is under siege by a alien substance. Which is why young men for example that have a very strong response, risked heart attack more.

    Wrong way ma'm, no massage is recommended,no aspiration and no holding muscles is required. Instead Z track method is recommended for IM

    I do Intramuscular injections on myself for Testosterone. I’m still a little too scared to do it into my arm, but seeing a doctor do it that quick and easy is relieving. I take like 10 minutes just preparing myself to take it 😭

    Nurses always pursue lifelong learning. The z track method was part of the EBP, therefore that's what CDC now recommends. =)

    1. Injection vial cleaning technique is wrong. Just a single wipe in one direction with swab is sufficient. Not as in this video backwards and forwards several times. 0.46 in the video. Introducing potential infectious material from outside the vial cap across the injection draw site.
    2. Z track prevents back seepage of medication and blood.
    3. Depending on the medication administration instructions. Some do not recommend massage after.
    Otherwise very good video.

    1. Aspiration is no longer recommended, neither is pinching the skin
    2. Use of the z track method is recommended
    3. Massaging the area increases the likelihood of solution entering the cutaneous layers instead of staying in the muscle
    4. Use a blunt fill needle to draw up solution so as not to dull the injection needle.
    5. You can engage the safety mechanism of the needle instead of recapping

      Aspiration should still be done with medicines like antipsych, it says on the individual medicine, if not can be a fatal mistake. its not recommended for vaccines but this is more a cause of the small volume and less harmful substance and making adult and kids go take vaccines without pain

    One critique is that the assistant did not explain drawing back on the needle to prevent injection in the veins? If she did get blood she would have to withdraw the injection.

      I use 2 finger breaths below the shoulder bone ( acromion process) to make sure we are in the deltoid muscle.

    Aspiration= good. Poor technique squeezing and too high injection site - placement is not into the bulk of deltoid muscle. Dangling bracelet = poor infection control practice.

    ok now I am confused. This is much easy but goes against what I was taught about IM injections. First, are IM not supposed to be above 1ml? Second, are you not supposed to use two needles for drawing and injecting. Lastly, are you not supposed to remove the needle and put on the cotton at th esame time?

      This video is very bad and I wouldn’t use any of it. High risk of skin reaction and infection if you do what she said lol
      First of all you should never pinch skin for intramuscular injection because you increase the distance between muscle and needle and there is very high risk that you will end up with subcutaneous injection or inject in vein. Is it bad? Not so much but high risk of skin reaction and inflammation if 1ml or so goes under skin.. also need to swap drawing and put on injecting needle..

      The deltoid muscle ca be injected 1-2ml for client from 3-18 year old above that age you can give also max 3ml in one single shot. you need really good reasons to give 3ml. The safe dose is 1ml then up to 2ml.

      Also I don't think you can shoot 1ml+ for people. 1ml is max, also it is good dosage for "larger people". For women and teenagers 1ml can easily be too much. EVen huge bodybuilders avoid more than 1 ml in delt

      For adult men 1 ml is fine, it depend on the muscle size, because of the pressure to around tissue

    This Nurse WAS "safe" as per what I was taught. I especially liked she tried to keep the slant of the needle UP. I'd be happy for her to inject me with any "Non-experientals."

    As someone that made hundreds of injections I wouldn't recommend using this as a guidance as you should ideally swap between drawing and injecting needles and not "pinch" skin/muscle before injecting.

      @@user-vx8te1id8r yes if you are giving injection to someone you can use "z-track method" and stretch a bit of skin without "pinching", that will reduce potential bleeding. But if you follow guidance in this video and "pinch" the skin, there is high risk that injection will miss the muscle and go into into subcutaneous layer, in most cases this is fine as you can inject under skin too, but it can give nasty lump for a week or so etc.. This is becuase when you pinch you elevate the skin further away from the muscle, so needle can simply become too short to reach muscle.. Pinching is only good if you want to inject into subcutaneous layer under skin

      Yeah I'm not a medical professional but I've had to inject myself for many years. Both IM and IV. There was definitely a steep learning curve where I would inject subQ by accident. I definitely thought the same things as I watched the video and totally agree with your statements

    Can someone explain what happens if there is blood when aspirating and what are the next steps from that? & why does the bevel have to be facing up? Thank you
