    What are the different types of hip replacement technologies?
    Hello, I'm Dr. Dan Albright with Raleigh Orthopedic Clinic.
    I have a lot of patients coming in and asking: "What's going to go in my body?" Are you using a plastics hip? A ceramic hip? Metal hip? It's a good question.
    This is a hip replacement model here. The first part we focus on is the stem. This is the spike that goes in the middle of your bone. Your bone is a cylinder and we put a stem in there. It can be cemented, it's less commonly done there, or uncemented. Then there's a socket, a metal socket, and a plastic liner or a ceramic liner, or a metal liner.
    I wanted to go into some different types of shapes. This would be an example of a stem that would go down the femur for a redo, a revision, a complicated surgery. This has a sandpaper type of material, a porous coating on it. The bone grows into it. Very, very effective. Over six weeks your bone literally interlocks, biologically interlocks, with the metal. Very solid. This is cylindrical there are also tapered stems, fluted stems which have ridges. This is the famous S-ROM stem with flutes on it, a little crease in there for stress transfer. Different shapes, they're modular, they come apart.
    Here's the point for my patients, whatever goes in needs to be road tested. A lot of people want the latest and greatest but our technology goes two steps forward and one step back. And that's a big deal. There have been a lot of recalls with technology. So you get a hip replacement and suddenly you have to have it removed because it has been recalled. It's failing.
    So what you want in there is what's been out for years. That's an important point for me. I put in implants that have been out 10-20 years and have been proven. If it's been out six months or less you want to wait.
    But anyway there's a lot of exciting materials that we use - cemented stem, uncemented stem, porous ingrowth. Different shapes we use depending on the anatomy and depending on your problems.

    Is cobalt still used? Can any of these hips cause brain tumors or any known cases?

    An egregious omission here is the absence of mention of a hip resurfacing option. Hip resurfacing, not to be confused with hip preservation, involves a much smaller implant that has a significantly reduced overall exposure to the patient for metal ion particulate contamination (metallosis). Hip resurfacing takes the surgeon longer to perform, which is why most orthopedic surgeons opt for the much faster total hip replacement. Revision surgery with a hip resurfacing is less statistically prevalent as compared to the failure rates of total hip replacements. People with metal sensitivities are much better off considering a hip resurfacing instead of a THR because of the large surface area of a THR implant that is not coated to prevent ion release will result in a THR damaging the surrounding tissues, resulting in a failure of the implant. We need the FDA to mandate an implant registry in the United States. Implant manufacturers lobby against said registry because the data that would be obtained from the registry would prove that THR's fail at an abysmal rate. Its easy to prove - simply look at a country with implant registry's and the data is clear - THR's are a disaster.

      its true but why is Hip resurfacing not been recommended by doctors. like if patient is ready to pay they still hesitate.

    Should you use ceramic and plastic instead of metal since cobalt and nickel toxicity in blood & urine sample? Will that give like Symptoms of dementia or Alzheimer’s?
