The science behind Ketamine as a possible cure for depression - Albert Dahan

  • Like many other drugs, ketamine has multiple effects rendering it suitable for various indications including anesthesia, sedation, acute and chronic pain relief, and treatment of therapy-resistant depression.
    Ketamine is a complex drug with a high affinity for various receptor systems including the NMDA receptor, opioid receptor system, and cytokine receptors such as the Innate Repair Receptor system (IRR). The activation of multiple receptor systems is translated into a variety of wanted effects that are useful in clinical practice.
    Clinical effects include anesthesia due to a dissociation between the thalamus and limbic system (ie. Ketamine is a dissociative anesthetic), relief of acute pain due to inhibition of excitatory NMDA receptors at spinal and supraspinal sites, relief of chronic pain due to activity at NMDA, opioid and cytokine receptors (e.g. the IRR), rapid antidepressant effects, due to the enhancement of synaptogenesis at prefrontal cortical areas.
    Notorious are ketamine’s side effects including addiction, psychedelic or schizotypical effects (hallucinations, paranoia, anxiety/panic attacks), cardiovascular, liver, and urogenital toxicity.
    In clinical practice the occurrence of psychedelic effects is worrisome and various approaches are being developed to counteract these effects. As a recreational drug or drug of abuse, ketamine is prone to addiction and development of damage to various organs (liver, the urogenital system). Research in volunteers and patients in the LUMC has led to a wealth of information on ketamine’s analgesic and antidepressant behavior which will be discussed.
    Dr. Dahan is a Full Professor of Anesthesiology at Leiden University Medical Center. He graduated from the Free University (VU) in Amsterdam (MD) and Leiden University (Ph.D.). He is head of Research at the Department of Anesthesiology. He founded and heads the Anesthesia & Pain Research Unit in 1995, a non-profit academic institute that performs outcome research in anesthesia and pain management aimed at improvement of care in anesthesia and pain treatment and the wide distribution of gained knowledge, in close cooperation with non-academic partners. The research on opioids and ketamine are among the major research topics of the Anesthesia & Pain Research Unit. With respect to ketamine, these studies focus on wanted effects (eg depression and pain) and (psychedelic) side effects. Prof. Dahan has published over 300 papers in peer-review journals. ***********

    I just started ketamine therapy. I've only done my first dose. It's one dose a week for just over 4 months.
    I felt... NORMAL... For a non-suicidal human being for a solid 3.5 days. Then it sorta wore off.
    I'm really looking forward to my next 17 doses and my integration therapy.

    Thanks for a great introduction to the uses/effects of ketamine.
    I'm confused by some of the comments about this researcher being uninclined to self-experiment with ketamine. Do people expect their oncologist/surgeon/obstetrician to have undergone an unnecessary treatment of radiation, chemo, a surgery or a C-section in order to be qualified to administer that treatment? No.
    Someone also commented that "His fear of the psychedelic effects makes me roll my eyes." I think the Dr pretty clearly explained that he's seen enough bad trips to know that they are something to be afraid of - but is still championing the therapeutic uses of ketamine, as well as further research. He also said that if he had a medical need to use ketamine, he would take it.
    I like that he has an understanding of the good and bad potential effects. That's what you want in someone who's running studies, or treating you. It doesn't sound to me like he would have anything much to learn from self-experimentation that 10 years of administering it first hand and listening to his subjects and patients hasn't provided.

      _I think he's just trying to keep his reputation squeaky clean it -- he's a doctor. He has hundreds of colleagues around the world. At the same time though, he gave a nonverbal indication -- he rolled his eyes -- when he said he has never tried it. The audience laughed. To me, he seems like a walking ketamine molecule, free from anxiety and depression, free to be himself. He is an outstanding physician by the way_

      @@Ketannabis it’s not supposed to be anywhere near effective as IV from what I’ve heard. Maybe more of a maintenance booster but IV is supposed to be superior for effectiveness.

    Thank you, Professor Dahan, MD. for such an informative video presentation. Concerning, Depression and Chronic Pain, these conditions are difficult to treat in addition to being very serious issues and I'm confident you have a heightened concern over them as well as I. And as far as Ketamine's concerned, it's a very good anaesthetic that's been around for years but every aspect must be respected, as well as your knowledge of it and other substances you're familiar with. I truly appreciate your frankness and how thorough you covered this subject. I believe I could greatly benefit from this, but for one, my needs would have to be addressed by a Dr. that's as knowledgeable as you, some of my needs would be difficult to address in my country, and most importantly, one has to be careful because Ketamine could harm certain organs and equally important, I personally feel it should be carefully administered and monitored by a professional such as you. Best of luck to you Dr Dahan and much gratitude.

    I had my 3rd ketamine treatment yesterday and feel so raw today the day after almost suicidal. It’s been waves of feeling better then almost great then horrible again. Basically I’m scared.

    A large part of my life was in a hospital emergency department. Ketamine is an extremely powerful drug and I've never seen a prescription written for it. I have had fentanyl once for a heart cath and have since refused to repeat that experience.

Although the big chain clinics try to force you to pay up front for multiple infusions, I found a less greedy one that let me have 2, back to back on Thursday and Friday, and then boosters every few months. That is what has worked well for me.

    I am afraid to take Ketamine (the speaker actually said he would not use it himself -I'm not sure if he meant he would never volunteer or would not try it if he needed it for a health condition) My Pain management doctor recommends ketamine infusion in low doses for neuropathic pain. It is given in a clinical setting. Did anyone use it for NP? How did it work?
    Is it as "frightful" as Dr. Dahan says..?

      I personally did not enjoy the dissociative feeling from IV Ketamine Therapy. You build tolerance to it and luckily I don’t get it anymore. In honesty when you first start treatment, some experiences can be a bit scary.

      It’s amazing that he says all these “facts” but I took 6, 90 minutes treatments and did not have any negative effects and my chronic life long (68 years) depression is gone and am off SSRs. I took pcp in college years 45 years ago and it did not affect me in any way like ketamine did this year. My arthritis in my hands is about 30% gone and I can work with my hands again. I find it interesting this guy won’t take it but of his negative beliefs about the drug. I wonder if he has ever experienced depression or ptsd? I’m not finished watching yet so I’ll get back to it. I’ve met a doctor that’s never had depression that prescribes ketamine for depressed patients with great results.

      @@inhalefarts In all likelihood the antidepressant effect is both biochemical (NMDA receptor, stops glutamate excitotoxicity, etc.) and 'dissociative' - which is really a more apt description for the mental state of low dose K than 'psychedelic'. You don't see little monsters crawling all over the walls of the room you're in or whatever crazy shit people see on shrooms, DMT or LSD. You see warm glowing lights and tunnels or visions from your life. It's like a pleasantly amp-up regular dream.

      Look for an MDMA dealer and he might have or be able to find a Ketamine source. MDMA is much easier to find than Ketamine so that approach would be your best bet.
      An easier and probably slightly saver method (purity) is to order it from a dark net market, where you can easily find it. Street Ketamine (but also dark net Ketamine) can be impure and / or a drug cocktail. It might also not even be Ketamine.
      The hardest way is to get it by knowing or being an animal vet (most of them use it) or a doctor. But it's very hard to steal it when you work there. What I've also heard of is people stealing hospital trash bags and with some luck scoring slightly expired Ketamine and a whole bunch of other drugs.
      I'm just informing you and other people, I'm not telling anyone to commit a crime, especially not people who might abuse it. Anyways, Ketamine might be legalized or get tried in pilot projects where certain people can apply (depending on country). If you have depression you might be lucky enough to have it administered to you soon! I advise you to search the web; maybe you can sign up for such pilot projects already.     Just wondering out of those here that have posted and used it, what country are you from? I'm in US, and is so terribly frustrating trying to get pain relief. Chronic pain truly begins to mess with your mind in a serious way. When anger sets in because no one will help you, well that is a very dark and dangerous place to be when the doctor is so uncaring that he wont HEAR you. With a stroke of his pen he could give you your life back....and wont.

      I would HIGHLY not recommend doing that. You would need to have it safely infused in a clinic where you can be monitored. If you use pills or powder (and buying it from some unknown source - you don't know if it's pure, etc.)...you are opening yourself up to the risk of dependence and addiction. By having it infused in a clinic by professionals, you are getting the benefits safely.

      I’m not sure I agree that he should be required to take K to study it, but especially if he doesn’t suffer from depression/anx. These disorders are caused by skewed chemicals in the brain so it stands to reason that K would have a different effect on those who lack what they need chemically and those who don’t. Just as non-diabetic endocrinologists aren’t required to take insulin to study and understand it’s clinical effects on diabetics.
