I think what's happening during the omega runs where Mausoleum II doesn't spawn dads note, is because you kill moms heart turning it into the Dead Mausoleum (I'm not sure what the floor is actually called), then on the victory lap for some reason the game thinks you're taking the alt-path, and already killed moms heart rather than taking the beast path and spawning dad's note. Just guessing but like you said the game wasn't designed for something like an omega run.
Also to note, that music in the dead mausoleum is the same music that is in the mom room when you kill mom's heart via alt path opening corpse, before you jump into corpse 1, it's that same sort of sad issac theme music that's echoed/reverbed
I believe that's one of the mechanics of sin. At the beginning of the Run, I believe that Sin started with two black hearts and deliver only gave him half of a soul heart
at 1:27:57 he loses half a heart from picking up the body. just a guess, but maybe the way the creator coded red heart upgrades works like this: give the normal amount of health given (for the body, it's 3 soul hearts) and then subtract half less than that amount (3-2.5). If the health bar had no limit this system would work perfectly fine, but because there's a 12 heart cap, 10+3 is rounded down to 12 first and then subtracting 2.5 gives 9.5 hearts. I think soul heart items give either half or a normal number of hearts (i wasn't paying much attention to those lol), and all red heart upgrades give half a heart, but that's just a guess. there's probably an actual explanation on the mod page. EDIT: from the mod page: "Sin starting Health can be Black Hearts from 1 to 3 - including half hearts (same Game Seed will have same health). Additionally, Sin cannot get any Red, Bone or Eternal Hearts. bonus that gives Red/Bone/Eternal Hearts will give instead a fixed Half Soul Heart instead" I think I'm right.
jeez, that “dead mausoleum” glitched floor is something else, just the ominous corpse ambiance on a dark floor with no exit. makes me think of old creepy pastas or the like. an interesting story for sure

i think with "clear all rooms", it means, you must kill all available enemies on the floor. Accordingly, rooms without enemies do not have to be visited. btw i seems with all hp-upgrades you got half the amount of hearts, you could get at a normal run.
Hi Sin, I think the reason that Dad's Note didn't spawn is because you didn't fight mom and used the negative/polaroid acquired in previous runs. At least that happens to me 100% of the time when I do a victory laps and go for the Beast kill at the end.

I drove 9hrs cross country to see some family so no Isaac adventure posts for awhile :) still enjoying your videos, and even after over a year of watching I still learn something every time I watch. This episode I learned sissy can make Gemini attack eachother. I thought they were the same entity
My favorite part of this was during the Beast fight, where Sissy is just hanging out in the lava. Her head popped up above it every so often just like: *_*

might be less to do with the victory lap itself and more that the game isnt designed to handle both happening in one run
