I think that's impossible, like if you spawn on dragon island you wouldn't be able to kill the dragon nor leave the island and if you spawn on another island and there's no end city with pickaxes you wouldn't be able to leave it. : P

At First the Dragon would have to Knock you up to each End Crystal, after you destroyed all of them you would have to kill the Dragon by hand. Or You spawn on the outer End Island. It would be much much easier. Just find an End city with loot, then locate a Endgateway (they spawn randomly) and use it with an Ender pearl. The Outer-Endgateway teleports you back on the Main End Island. After that you just have to kill the Dragon normally.
