Unboxing the components of an anterior total hip replacement surgery

  • Dr. Dan Lieberman unboxes a Stryker total hip replacement implant. The usual hip replacement has four components.
    The stem is the Stryker Accolade II. The trunion mates with the ball to make a cold weld to hold the implant together. The body has 2 parts, a hydroxyappetite coated upper part allows for bone growth. The shape of the body was determined by CT reconstructions for a tight fit.
    The cup is the shell which holds the ball.The cup is also covered with hydroxyappetite to allow bone growth into the socket. There are holes to permit a screw to temporarily fix the cup into the acetabulum. Thinner is better for a shell because it allows a bigger ball, which reduces the risk of dislocation.
    The cup is lined by a polyethylene liner. The plastic is radiated to make it stronger.
    The Ceramic head fits inside the cup and replaces the ball. Ceramic is better than metal because it wears better according to registry data.

    Just had my anterior hip replacement on 9/27/23 with the same type of components (not sure of the brand) and the recovery was amazingly fast. But what's even more impressive is the smooth natural feeling of the components when walking. I had heard from a friend who had the same surgery a few years ago that the only thing he didn't like was the unnatural feeling of the ball and socket. Mine feels so natural that I can't tell the difference between hips. So glad I had the surgery.

    Hi Dr Lieberman. I have watched your videos about spine surgery now am looking to get myeft hip done soon. Had anterior procedure done on right hip 15 years ago. Now having a hard time finding a Dr that will do it seems they all want to do posterior and I don't want that. My original surgeon has retired.

      Awesome! We hope you are getting a robotic assistant, outpatient setting with spinal anesthesia, and registry proven implants. What approach does your surgeon use --- Anterior, direct superior or superPATH?

      NONE YA I had my replacement surgery 11/28/22. Recovery is going great. Should've done it long ago. Wishing you a smooth surgery and speedy recovery.

      I'm having a bilateral hip replacement ( both at same time) In a couple of weeks with Stryker accolade II. Great stuff, now I now what's going into my body.

    I have a Stryker Tritanium from 2010. No problems as far as I can tell. Is that cross-linked polyethylene? Do I have cross-linked polyethylene?

    I had both hips replaced in 2021, they are working great. I had a question that I could not find an answer to.
    I know they say not to use titanium in tooth fittings as they act like an antenna and can increase EMF, and 5G, how would having Titanium hips affect the body? Would it do the same as having it in the mouth and act like an antenna?

      Hi, Mark. Sorry for the slow reply. J&J is great. They are one of the big 4 makers of hip replacements. I would pick the doctor and let them pick the hardware; however, I wouldn't pick an orthopedic surgeon who uses crappy hardware. Please let me know if this helps!

      Yes! There are 2 sizes of necks. The neck length has a huge impact on the construct, as you might imagine. There are so many combinations of parts that make up a construct! Fortunately, the robot helps these orthopedic surgeons sort it out. There are some surgeons I really respect and have seen getting great results who don't use the robot fir hips; but I don't think I would have surgery without a robotic assistant.

    HI, WHAT ABOUT OXINIUM (oxidised zirconium hip stem head surface layer) used by smith & nephew on their Polar 3 implant. Would you opt for the ceramic? Tests show lower wear using OXINIUM with highly crossed PE over 5 yrs. Bottom line would you go for OXINIUM or ceramic? I am deciding for myself soon to be operated on.. Thanks:-)

    Hello, is there a sleeve that goes into the ball than you get that tight weld??

      It's a cold weld done by friction of the fit. I find it amazing that this type of joint is nearly as strong as an actual weld. You can't use flame in the operating room as the gasses are flammable.

    I'm reacting to the co cr liner in the other model it looks like they can just take out the liner and put in an oversized bearing ?

      No. They need the liner to avoid metal on metal debris from forming in the hip.

      @@hip-replacementinfo6511 no the liner in the dual mobility hip basically making it a constrained polyethylene like the one in this video I see where they can. My new question is can they use an adaptor sleeve and put a ceramic ball back on instead of using a cocr ball ?

      More like 700$ for Stryker.. I don't know how much they sell the actual components in the States but building price is surely less than 1.000.. Also one more thing I am pretty sure that the acetabulum has a titanium porous coating and not HA..
