Apparently Ripley and Kynan were illusions from Major Image and any damage taken was just them using their reactions to change the image. The party just wasn't close enough or had the INT/Investigation to see through it.
The illusion was probably the spell “programmed illusion”. It carries out a set of commands given to it by the creator, and exists to act as a big target to distract other people.

She's a high level Wizard/Gunslinger if I remember correctly, so she should be able to do a major illusion or better.
